We currently have a vacancy for a Community Nurse, working with the Liskeard-Looe Community Nursing Teams and to work flexibly across the East locality when required. Situated in the beautiful East Cornwall we are busy, dynamic and forward thinking.

We are currently seeking to recruit a Community Nurse, with a commitment to provide compassionate, holistic patient care, working flexibly across the area.

You must possess excellent communication and people skills have the ability to work autonomously with little direct supervision, whilst being a committed member our supportive team.

Ability to travel independently across the County to fulfil the requirements of the post is essential as this position will be visiting our housebound patients providing nursing care in their own homes.

A use of a vehicle is essential.

This is a lone worker post.

Main duties of the job

Responsible for the assessment, planning, implementation and evaluation of care, without direct supervision, to patients, carers and relatives within their home environment, enabling them to enjoy maximum independence and optimum quality of life.To work in partnership with patients and carers to offer support and advice and promote informed choice. Responsible to support the role of the Community Nurse Team leader.To work autonomously, providing effective, evidence-based care in line with competency and training record and organisational policies, procedures and guidelines.To support all locality teams by working in a flexible way. Moving to work with the team in most need on a daily, weekly or monthly basis as service provision demands.
