As a Children's Gastroenterology Clinical Nurse Specialist, the post holder will act as an expert nurse and role model in the care of children and young people and their families.

The post holder will demonstrate a good understanding and proficiency in the highly specialist nursing care and management of children and young people in both the hospital and community setting.

The service manages a comprehensive range of Gastrointestinal- Nutritional disorders the caseload will include children with inflammatory bowel disease, liver, coeliac and oesophageal disease including, feeding problems and functional bowel disorders.

Main duties of the job

A key role and interest in care of patients with inflammatory bowel disease ( IBD) .

Co-ordination and care of the young people having biologics treatments

The post holder will also participate in clinical work and coordination of tests within the new clinical investigations unit in liaison with the Gastroenterology team.

They will make recommendations and identify areas for development and present these to the gastroenterology team for discussion prior to implementation.

They will be responsible for the development, implementation and evaluation of programmes of care, with support from the Consultant Paediatric Gastroenterologists and Matron as the service dictates and ensuring all activity is recorded as required.

The level of clinical judgment and autonomous decision making will depend on the individual post holder's level of experience in specific areas.

Job responsibilities

Communication

Develop and initiate strategies to effectively communicate highly sensitive and complex condition related information to children/young people and their families to enable informed choices (e.g. life-long nature of condition and long term complications).

Promote effective communication with children where age or disability are barriers to communication and require particular skills for this to be achieved.

Implement and evaluate support given to families who may be distressed, shocked, grieving or occasionally aggressive as a consequence of childs illness, social circumstances, e.g. new diagnosis, safeguarding children.

Articulate status of children to the multi-disciplinary and multi-agency teams as the childs status dictates.

Report child protection concerns to the appropriate agencies.

Demonstrate use of IT skills to complete details of admission/discharge/ visits of patients using a computerized record system

Write precise, accurate and timely nursing records, reports, statements, referrals and letters.

Listen actively and respond appropriately to information given.

Promote and develop effective communication with members of multi-disciplinary/multi-agency teams, identifying options and actions for co-ordinated packages of care.

Provide detailed telephone consultations and initiate action as needed.

Provide constructive feedback to individuals and the team.

Participate in case conferences and discharge planning meetings.

Recognise when own and others behaviour is not acceptable and initiate action to resolve effectively.

Initiate and provide support/advice to parent carers over the phone. (eg. Inflammatory bowel disease patients liaison about Azathioprine blood monitoring tests )

Develop effective communication with Adult Gastroenterology Teams responsible for young adult care and contribute to effective transitional care from child to adult services.

Service Delivery and Improvement

Manage and co-ordinate caseload autonomously.

Interpret and analyse information from a range of sources to underpin decision making, reporting to gastroenterology team as required.

Identify needs, review and participate in the ongoing developments of the Childrens Gastroenterology team.

Work within agreed resources as part of the paediatric gastroenterology team

Act as a role model for junior members of the Team and non-specialist staff

Participate in mandatory and other audit.

Facilitate the cascading of information from other sources to junior and senior staff.

Have awareness of organisational structure, the function of the Directorate within the organization and Trust objectives.

Participate in and promote others participation in Trust wide forums.

Provide supervision and mentorship as appropriate, identifying development needs and implementing strategies for individuals personal development

Contribute to recruitment decisions, team objectives, performance standards and plans of action

Maintain and raise awareness in others of the efficient use of all resources within the specialist area.

Facilitate the admission/discharge of patients to and from hospital making appropriate referrals when necessary in accordance with Trust policy.

Liaise effectively with colleagues, the multi-disciplinary and multi-agency teams and other departments to allow the timely management of the patient caseload.

Prioritise own work and others and reorganise continuously in the presence of unpredictable work pattern and frequent highly unpredictable interruptions e.g. responding to frequent bleep/telephone requests.

Recommends and participates in the service development and evaluation of specific childrens gastroenterology innovations across the Trust in order to meet national standards, e.g. NSF recommendations, NICE, Royal College. Monitor activities against policy standards

It is anticipated that the post holder in collaboration with the Gastroenterology team will help submit work for presentation at scientific meetings and publication.

Patient Care Delivery

Model excellent practice through effective assessment, planning, implementation and evaluation of evidence based specialist programmes of nursing care for children/young people and their families working alone flexibly in both hospital and community setting.

Deliver patient care autonomously, consistent with their level of expertise, using available technology, clinical judgment, childs records and parent/carers knowledge to identify changes.

Participate proactively in multi-disciplinary outpatient clinics (including peripatetic clinics), Day case unit activities (eg. endoscopy patients, biologics therapy infusions, clinical investigation unit tests ) and inpatients (eg. Gastro- Nutrition ward rounds). Undertake baseline physiological assessments. Adjust treatments as appropriate and following appropriate training.

Act as a source of specialist advice for professionals and children/young people and families on the specialist nursing care and holistic management of paediatric gastroenterology within BSUH and the regional Paediatric Gastroenterology-Nutrition clinical network

Initiate and implement counselling strategies and provide support and advice to children/young people and families to promote emotional well being.

Develop and sustain effective partnership working, co-ordinating and contributing to packages of care for children, young people, parents and carers. Initiate referral to multi-disciplinary/multi agency teams autonomously.

Articulate the decision making process underlying the management of own and others time and the patient workload.

Recognise organisational and clinical needs that may affect the area in which they work. Respond to these needs

Identify health promotion needs and develop specific strategies to meet the needs of the individual.

Articulate, critique and discuss evidence base for care delivered.
