We have a fabulous opportunity to be part of the continued development of the GP-led, Acute, Home Visiting Team in Derby City.
We seek an enthusiastic and forward-thinking Advanced Clinical Practitioner to join our established team to provide care for patients in their own homes.
We are looking for an individual who will work across GP practices.
The post holder will be an experienced clinician who, acting within their professional boundaries, will work flexibly to provide care for patients from initial history taking, clinical assessment, diagnosis, treatment to evaluation of care.
The clinician in this role will be key to supporting clinical work in the patients' home, you will have links to a wider network of multi-disciplinary professionals, whilst acting as a positive and enthusiastic role model, supporting junior staff, and developing our workforce.
Main duties of the job
The Home Visiting Service is a team of Nurses, Paramedics, Enhanced Clinical Practitioners & GPs, who work autonomously to deliver Home Visits for the GP practices within the Primary Care Network.
You will lead and monitor improvements to quality of care through clinical audit, evidence based practice, clinical supervision, teaching and supporting professional colleagues and the provision of skilled professional leadership.
You will lead the implementation of relevant clinical guidelines into practice and contribute to the further development of the evidence base through research and audit.
We offer protected time to undertake supporting professional activities including continuing professional development, mentorship and teaching of others, clinical audit and contribution to the wider work of the Trust.

- Organise, prioritise and schedule visits to homes and resident's dependent on clinical need

- Provides expert care and advice to a complex and dynamic caseload of patients presenting with a diverse range of medical conditions.
- Undertakes first line comprehensive clinical assessment of patients, including those with complex presentations, employing an extended scope of practice including advanced consultation and physical assessment skills.
- Requests, reviews and interprets diagnostic investigations within the context of other available information utilising a systematic process of clinical reasoning to formulate a differential diagnosis.
- Develops and records a treatment plan consistent with the outcome of assessment and the most likely diagnosis.
- Reviews and initiates medicines through independent prescribing or Patient Group Directions Involves patients, families and carers in the identification of patient-centred concerns and priorities about health and well-being and negotiates approaches available to prevent deterioration or promote comfort and well-being.
