East of England Provider Collaborative

Cambridgeshire and Peterborough NHS Foundation Trust is delighted to host a new commissioning team to support the new East of England Provider Collaborative (EoEPC). The new team will deliver improvements in care for people using specialist mental health services across the East of England region.

The EoEPC supports a population of over six million people, delivering specialist mental health and learning disability services.

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Main duties of the job

We are looking for someone who holds patient experience as central to their practice as a registered mental health professional.

The vital role of a Case Manager within the East of England Provider Collaborative encompasses both Quality Assurance and Quality Improvement. Ensuring that the care received by Children and Young People in the East of England throughout their journeys through specialist inpatient units is of high quality.

We are looking for an engaging, enthusiastic, and experienced CAMHS practitioner who can build robust relationships with people, and friends and family of people who have been admitted into Tier 4 Services and with a variety of other stakeholders in both statutory and non-statutory organisations.

As a Case Manager, the post holder will work as part of a dynamic team in ensuring our services and care pathways for young people are effective and safe across both statutory and independent sector providers.

The role supports the business of driving transformation as well as value for money in planning, and commissioning of services. The role is designed to build a combination of subject matter expertise and technical skills to help to develop strong service delivery across the East of England.

Job responsibilities

  • To provide clinical assurance of the quality of the care provided by specialised services within the east of England.
  • Managing individual patient pathways to ensure high quality services and care planning outcomes are achieved.
  • Provide proactive case management through effective liaison with significant stakeholders for the placement of patients according to evidenced clinical need and undertake monitoring of the placement through regular attendance at relevant CPA meetings and Section 117 reviews to ensure the effectiveness of treatment and efficacy of care packages.
  • To be involved in patient forums in order to appreciate the patient experience and address, with the Senior Case Manager and Commissioners, issues raised with regards to compliments, concerns and complaints.
  • Work collaboratively with all providers to ensure quality improvement processes are in place.
  • Take an active role in the development of partnership working across the east of England to promote timely and equitable patient access to services once assessed as having a clinical need and risk, and timely egress from services ensuring the most appropriate level of security/setting is available in accordance with the patients agreed care pathway. Identify and negotiate placements for patients at the point of initial referral.
  • The post will also include being responsible for ensuring the development and articulation of best practice across providers of the pathway.
  • Attendance and participation at local, regional and national forums related to specialised services.


Person Specification

Education

Essential

  • Registered Mental Health/Learning Disability Nurse or other registered health or social care professional
  • Educated to Post-graduate degree level in relevant subject or equivalent level qualification or significant experience of working at a similar level in specialist area.


Experience

Essential

  • Demonstrable specialist knowledge in a relevant discipline
  • Evidence of continued professional development.
  • Experience of managing risks and reporting.
  • Knowledge of legal issues pertaining to the specialist service, including MHA 1983, Ministry of Justice procedures


Skills and abilities

Essential

  • Ability to produce high quality and accurate reports.
  • Self-motivated and able to work independently.
  • Ability to work under pressure.
  • Ability to travel to disparate locations on a regular basis.
  • Demonstrated capabilities to manage own workload and make informed decisions in the absence of required information, working to tight and often changing timescales.
