We are looking for a Registered Nurse to join our dedicated and highly professional team of community nurses and therapists in North Norfolk. We have permanent positions available within the NN3 Team based at Aylsham Health Centre. Our service cares for individuals living in Aylsham and the surrounding areas.

The Community Staff Nurse role is a role under the supervision of Band 6 and Clinical Lead colleagues as part of the larger integrated team, providing high quality patient centered nursing care to patients in a variety of community settings. This is an ideal opportunity for those either wanting career progression and experience within a community setting or those with previous community experience, skills and knowledge.

You must be self-motivated, able to demonstrate sound clinical reasoning skills and have good communication skills.

Main duties of the job

You will demonstrate a proactive philosophy to care, committed to a whole patient approach, ensuring holistic care of the patient and involvement of all parties. You will be someone who believes strong and effective communication is essential and who strives to ensure quality patient care is their highest priority.

To assist you in doing this you will be clinically supported by our Community Matrons, Clinical Leads and by a management team committed to patient centered quality care. You will receive support and development through in service training, regular appraisals, a personal development plan and clinical supervision.

The Team in NN3 is key to the on-going development of our Primary Care Network, creating strong links between our operational services and our local GP practices. As an individual within our team, you will effectively communicate and advocate for those receiving our services with G.P practice, Social Services, Voluntary and Third Sector to ensure a joined up and outcome driven approach

Qualifications

Essential

  • Current NMC registration or awaiting PIN


Desirable

  • Degree or working towards degree
  • Teaching and assessing qualification


Experience

Essential

  • Experience working with multidisciplinary teams and partner organisations.
  • Experience of working with patients with multiple pathologies and managing holistic patient centred care.


Desirable

  • Appropriate extended skills e.g. male catheterisation, management of syringe drivers, venepuncture and ear irrigation.
  • Previous Community Experience
  • Previous qualified nurse experience
  • Previous palliative care experience.
  • Venepuncture and ear irrigation


Skills and knowledge

Essential

  • Ability to organise and prioritise workload independently, within a team and with wider integrated team
  • Knowledge/experience of the audit process
  • Knowledge of safeguarding issues relating to the care of older people
  • Ability to work on own initiative.
  • Good computer skills and the ability to use information management systems.


Desirable

  • Experience of audit/research
  • Relevant safeguarding training


Communication

  • Excellent verbal, written and interpersonal skills appropriate to supporting team members

  • Evidence of own personal development and experience
  • Able to support the development of others through supervision and teaching
  • Supporting patients in self-management through health coaching

  • Mentorship qualification will be essential but can be gained whilst in post
  • Health Coaching training

  • Must hold full and valid and driving licence and have access to a vehicle
  • Able to communicate effectively in written and verbal English Language
  • The post holder will be required to work seven day a week and cover between the hours of 08.00 and 18.00 hrs.
  • The post holder will need to be passed fit to perform full duties of the post through occupational health with any reasonable aids and adaptations if necessary
