With a background in health inequalities, the postholder will lead the Trust in the development, implementation, and delivery of our strategic plan. Taking action to reduce the health inequalities is one of three strategic quality priorities for the Trust and this is therefore an important and exciting role. They will be responsible for ensuring that the Trust is compliant with the Public Sector Equality Duty as part of the Equality Act across all of the services we deliver. The postholder will cultivate trusted and impactful relationships across the system and into our communities enabling and involving people who may be affected by inequality or exclusion.
Main duties of the job
They will lead the implementation of a health inequalities impact assessment system and accompanying processes in LYPFT - ensuring that this becomes an embedded part of business and usual in our service delivery, service development and business planning work.
This person in this role will understand how people with Mental illness and/or Learning Disabilities are disproportionally affected by health inequality. They will understand the complex drivers of inequality and intersectionality, ensuring that each of our services has an improvement plan to address the inequality faced by people with protected characteristics. Our focus on inclusion will be maintained and strengthened by the person undertaking this role - and they will be closely aligned with colleagues from our Workforce directorate leading the work on workforce EDI. This role will lead the development and implementation of the LYPFT addressing health inequalities long term strategic plan. They will be the coordinator of all of our addressing health inequalities activities across the Trust and known and regarded as the link for our Organisation across the health and care system in Leeds.

- Develop and implement the Trust Health Equity strategic plan, working with cross Trust Representation through the Health Equity leadership group and other members of the Trust to ensure that this effects demonstrable outcomes, in line with Trust values.
- Review, establish and lead the resources and governance arrangements needed to drive the health inequalities agenda in LYPFT.
- Be responsible for assurance on statutory, legislative and NHSE requirements on health inequalities.
- Create appropriate systematised measurement of impact and be responsible for data and information reporting in line with requirements.
- Work with our partners in the city, through for example membership of the Leeds Tackling Health Inequalities Group the Trusts Third Sector Partnership Group to achieve an impact on health inequalities. Representing the Trust at Leeds place and ensuring that we have appropriate and representative involvement in community groups.
- Work with colleagues at a West Yorkshire CB level representing the Chief Operating Officer and the Trust and responding to the requirements of the WY and HNY ICBs. Drawing in learning and creating networks to enhance our performance in addressing the gap.
- Work with the Patient Engagement and Involvement Teams to ensure that we appropriately enhance our community engagement in ways that are effective, to mitigate the risks associated with relevant protected characteristics and social and economic conditions, ensuring person centred care and effective engagement with those communities experiencing the greatest health inequalities.
- Manage the work on inclusion across LYPFT care services through the network of employed people specific to this agenda.
- Create systematised links with the Workforce diversity and inclusion team to ensure that staff across Care Services are equipped with the requisite skills and competencies to deliver on this agenda.
- Integrate the work of the Synergi collaborative into LYPFT supporting the wider agenda they have specific to the Leeds MH Population Board.
- Work with the Leadership Teams of Service lines across LYPFT to ensure that our services are developed and delivered inclusively and in way that best services those patients and communities at greatest need and that future transformation is done in a way that improves the health of those who need it the most.
- Work with the services to ensure they offer equitable access, great care and optimal outcomes to those experiencing the greatest health inequalities including developing digitally enabled care pathways in ways which increase inclusion, ensuring awareness of communication requirements (including health literacy/language/visual or sensory impairment) and ensuring face to face options continued to be offered.
