This is an exciting full-time opportunity for an experienced Advanced Assessment Nurse or Highly Specialised Physiotherapist / Occupational Therapist to join our dynamic and innovative team providing holistic assessment and treatment in a variety of community and acute settings, designed to reduce preventable hospital admission. Referrals to the service will typically be from General Practice, NHS 111, A& E, Ambulance Services or Community- based Health and Social Care (including care homes) as well as Acute Trusts and other rehabilitation in-patient centres. The service also facilitates two pathways from Secondary care - Discharge to Assess (D2A) that supports medically optimised patients being discharged in to the Community and Hospital at Home which supports patients being discharged home, who still require clinical visits and monitoring at home, under the remote supervision of a Medical Consultant and wider clinical team .

The aim of the service is to avoid unnecessary hospital admission by providing the Hillingdon borough population with a rapid (Within 2 hour) deployment of clinical response to patients in their own homes in order to make an initial assessment and form onward plan of care.

Main duties of the job

The team is comprised of Nurses, Physiotherapists, Occupational Therapists of varying experience as well as support staff including Nursing Associates , HCA's and Admin. Staff are able to access GPs, pharmacists, social services, dietitians as well as other specialist nursing and therapy services located in the borough as required, to support patient care.

The service runs from 08:00-00:30 including weekends and bank holidays. Typical Long-Day shifts run from 08:00- 20:30 and 12:00 - 00:30. We also support a 09:00-17:00 shift. Successful post holders must be able to work flexibly to include weekend and OOH working. Part-time may be considered for the correct candidate. As well as working in the community, successful candidates may be expected to support the Community Post-Covid MDT service.

Candidates should have previous experience working in the Community setting and will need the ability and clinical skills to work autonomously but to also work alongside other members of the multidisciplinary team.

  • Be able to work independently and as part of a team
  • Use their clinical initiative but also take direction from medics
  • Understand escalation pathways and safe working practices
  • Have a clear understanding of Safeguarding and Mental Capacity
  • Have the flexibility to work weekdays and weekends
  • Be friendly, caring, compassionate and adaptable

Possession of a UK driving license and access to a vehicle for business purposes is essential.

Education / Qualifications

Essential

  • RGN / Degree/diploma in physiotherapy
  • At least one clinical post registration qualification i.e. oSpecialist Practitioner oPhysical assessment course oRecognised diploma oITU, CCU, A and E oNurse Prescribing Course
  • Assessor/Mentor


Desirable

  • At least one clinical post registration qualification i.e. ? Specialist Practitioner ? Physical assessment course ? ITU, CCU, A& E ? Non-medical prescribing Course


Experience

Essential

  • Extensive Community experience or other comparable/transferable * Clinical roles.
  • Experience of managing others including PDP & appraisal.
  • Experience of sensitively managing & resolving complaints
  • Knowledge of NMC/HCPC/NICE Guidelines & current Government thinking.
  • Experience in supervising staff
  • Experience in working in a multi-disciplinary team
  • Multi-agency working
