Job summary

The Neuro Muscular Care Advisor (NMCA) has responsibility for the coordination of specialist, complex support of patients diagnosed or suspected of having a neuromuscular condition.

The NMCA will provide a point of contact for other health professionals involved, assisting with advice and navigation of the services available, coordinating access to ongoing assessments and services, and to ensure the patients care objectives are met in order to support patient choice.

This is an exciting post, suitable for a therapist (occupational therapist, physiotherapist, speech and language therapist) or nurse, to work seamlessly with patients across specialist and community care environments.

There are currently two posts advertised with an ability to offer full or part time working.

Main duties of the job

To provide care co-ordination across Swansea Bay and Hywel Dda University Health Boards for patients with neuromuscular disease (NMD) in line with the standards of care for neuromuscular conditions.

Neuromuscular disease (NMD) is an umbrella term for more than 60 rare and very rare progressive muscle-weakening and wasting conditions, such as Muscular Dystrophies, Myopathies, Inherited peripheral neuropathies, Metabolic diseases and Spinal Muscular Atrohpies.

Qualifications / Knowledge

Essential

  • Registered Nurse / AHP Practitioner
  • Evidence of Continuing Professional development and application to practice.


Desirable

  • Master's degree or evidence of equivalent in terms of knowledge/experience.


Experience

Essential

  • Expert knowledge and experience of management of patients with NMD
  • Significant post registration experience
  • Ability to Understand the role of Research and Development and utilise these in daily work.
  • Significant experience in Chronic Condition Management, Neurology or Palliative Medicine
  • Working within the community and hospital settings
  • Development and implementation of protocols/pathways


Skills

Essential

  • Able to demonstrate clinical competence to demonstrate clinical credibility
  • Excellent communication and interpersonal skills (to engage in distressing and sensitive situations)
  • Advanced patient assessment skills
  • Ability to influence staff at clinical and strategic level, and promote and monitor change.


Desirable

  • Ability to speak Welsh
  • Research skills


Specialist Knowledge

Essential

  • Sound knowledge of multi system disease management
  • Experience in development and implementation of protocols/pathways


Desirable

  • Advanced patient assessment skills in this speciality
