We are looking to recruit a Band 6 Clinical Nurse Advisor (Triage Nurse) for the Community Heart Function Specialist Team, based at Coley Clinic in Reading. You will be an enthusiastic and experienced nurse, with a warm personality and great people skills.

Willing to undertake relevant training, you will have leadership experience and be able to demonstrate innovative practice. Showing effective management skills, you will take responsibility for case management of patients with heart failure in a community setting. The role encompasses problem solving, caseload management and urgent patient visits.

Working as part of a wider integrated Cardiac and Respiratory Specialist Services (CARSS) covering the Berkshire West locality you will work to improve the lives of patients with Cardiac and Respiratory conditions. You will be responsible for ensuring high quality evidence-based care for patients with heart failure, their families, and carers in the community.

Main duties of the job

o The Clinical Nurse Advisor (Triage Nurse) will provide clinical support, advice, assistance, and professional expertise to all callers to the Heart Function Service, including both healthcare professionals and patients within Berkshire West. Make differential diagnosis within level of competence, plan and provide treatment in collaboration with others as appropriate

o Will offer clinical triage and support where appropriate. This may range from signposting the referral co-ordinator/caller to the appropriate service, making a direct referral

o To manage the Telehealth system (Virtual monitoring), making suggested clinical management plans in medication titration as required

o Supporting colleagues in the delivery of compassionate, structured care, safety netting patients and forward planning future appointments

• RGN
• Completed/completing first level degree
• Relevant experience in long term conditions and dealing with complex, multifactorial medical conditions
• Basic Life support skills

Desirable

• Heart Failure qualification e.g. Optimising Health & Wellbeing for the Patient with Chronic Heart Failure (Caledonian University) or similar / or willingness to undertake
• Nurse Prescriber / Nurse Independent Prescriber qualification
