Job summary

Are you a well organised individual who has excellent communication skills and time management? Do you have experience with discharge pathways within an acute hospital setting and looking for a new challenge?

We are looking for a Therapy Patient Flow Coordinator to join our dynamic team of Physiotherapists and Occupational Therapists where you will be the single point of contact for Therapy patient discharge related escalations.

Interview date will be Monday 5th June

Main duties of the job

To act as a contact point within Therapies for queries regarding discharge planning from clinical and non clinical MDT and Health and Social care partners.

Facilitate and support effective and early communication between hospital staff, patient and family regards to discharge planning.

Liaise with clinical and non clinical teams across the Trust via telephone, email, face to face and remotely where appropriate.

Escalate patient queries, capacity problems and general issues to relevant clinical staff as appropriate.

To attend and participate in relevant MDT meetings working closely with inpatient therapy teams to facoilitate flow throught the acute sites,action any escalations and to make or amend referrals for onward services.

To liaise with and refer to therapy colleagues and other external agencies to support the best possible management and discharge planning of patients.

To communicate complex patient related information effectively, to ensure collaborative working with the multidisciplinary team members across health and social care sectors, ensuring the delivery of a co-ordinated multidisciplinary service.

Job responsibilities

The right person will be organised, motivated and able to work under their own initiative. You will have excellent communication skills and be confident liaising with other health and social care professionals as well as patients and relatives/carers. You will represent Therapy at relevant meetings and be lead contact point for therapy related discharge queries. You will also be required to collect data for the monitoring and evaluation of the service.

You will be supported by a Therapy Team Lead through a well-established supervision and appraisal process.
