Main duties of the job
Working alongside other MDT members & senior social workers, you will have the opportunity to develop skills in care coordination, social care commissioning, safeguarding, mental health assessment & treatment & much more.
We manage people with severe & enduring mental health problems in the community as part of one of the largest mental health & learning disability providers in the UK.
In this post you will be responsible for the delivery of care to an allocated caseload of clients living in the Community experiencing, or recovering from, mental health difficulties.
You will provide direct support and advice to Service Users (Adults of all ages) and Carers, within the recovery framework, embracing the individual right to choice and control. You will ensure that appropriate health and social care services are commissioned as required. You will also assess, identify and evaluate needs through comprehensive assessments, reviews, care planning in conjunction with other professionals.
You will work as part of a multi-disciplinary Team, working in partnership with a range of stakeholders inside and outside of the Trust.
