An opportunity has arisen for an enthusiastic registered nurse to join Gladstone Ward based in Deeside Community Hospital.
Deeside Community Hospital is a great place to work and feel part of a close community hospital team. The team enjoy a close working relationship and support from the hospital league of friends and a range of outpatients services also take place within the hospital site.
Gladstone is a 20 bedded ward currently surged to 28 and is supported by a Care of the Elderly medical team with Advanced Nurse Practitioner support and has additional support from therapies to develop agreed rehabilitation plans with patients, with the focus on discharge to recover and assess.
Main duties of the job
Patients admitted to Gladstone ward are transferred either from acute hospital wards when ready for rehabilitation or from the community to prevent the need for an acute hospital admission. Patients may require complex discharge planning, including the completion of long term assessments. Working as part of the nursing team, you will have the opportunity to develop excellent skills in patient assessment as part of their recovery as well as a range of nursing interventions. You will also work closely with community teams as part of the discharge planning process.
