We are looking for innovative, enthusiastic, and patient centred consultant geriatricians to join our team in the Care for Older People at Barnsley Hospital NHS Foundation Trust.

We are a friendly, dynamic and diverse team of experienced clinicians who will provide mentoring and support tailored to your specialist interests, and who recognise the importance of a good work-life balance.

We have a long tradition in multi-professional training and education at undergraduate and postgraduate level, as well as guiding candidates through a successful CESR application.

We are actively involved in care transformation for older people of Barnsley at Integrated Care Board level through the development of a Frailty Virtual Ward, Frailty SDEC, and Integrated Care with community colleagues, as well as service development and delivery in hospital inpatient and outpatient settings.

Main duties of the job

The successful candidate will be responsible for the development of Care of the Elderly services along with their colleagues and is expected to deliver a daily inpatient ward round as well as outpatient clinics and timetabled sessions on Frailty SDEC.

They also expected to have a strong commitment to participating in teaching and clinical supervision of medical students and trainees, contributing to the training of nurses and other staff and participating in the activities of the Postgraduate Medical Centre including Medical and Clinical Audit.

The candidate will have allocated 1.5 PA for SPA time.

This post will help to continue to facilitate the progression of Care of the Elderly services. It is an exciting time to be joining our team we are ambitious to further develop the model of care for frail elderly patients including our services in areas such as Stroke, Frailty, Orthogeriatrics and Dementia and we are keen to support the successful candidate in specialist interest areas.

Job description

Job responsibilities

An exciting opportunity has arisen to join the dynamic Care of the Elderly team and to participate in service transformation for the future. Our Care of Elderly department provides outstanding geriatric services to 4 medical wards including our Acute Stroke Unit and Acute Frailty areas.

We have a very innovative and proactive frailty team who have developed an Electronic Geriatric Assessment tool, as well as a Frailty SDEC area and front door frailty service in the Emergency department and CDU.

We are looking to transform the current structure of our Care of Elderly bed base to include a bedded short stay frailty area (to allow direct admissions to the department allowing early assessment by geriatrician), and a longer stay area (for admissions >72h). The successful candidate will have the opportunity to participate in the frailty service development, and deliver clinical sessions with the Frailty team.

We also have a Stroke Unit within the department that provides specialist inpatient care, TIA and stroke clinics. The successful candidate may want to have/keep experience in stroke management, and could be offered clinical sessions in Stroke if required.

The department has an outpatient area (Assessment and Rehabilitation Unit) with a long tradition of providing multidisciplinary care for patients with falls, incontinence and Parkinsons disease. Besides Parkinsons and Falls clinics. We also run tilt table, syncope, continence, general internal medicine and geriatric medicine clinics.

We have a specialist continence team, falls prevention team, dementia nurse specialist, dementia and delirium lead, and there will be a scope for cooperation with the successful candidate to improve those services.

We are keen to continue to develop our outpatient care and the successful candidate is welcomed to participate in outpatient care redesign and delivery to suit the specialist interest of the appointee.

The department also has a vibrant and developing Integrated Care Service which provides a Virtual Frailty Ward service (step up from community and step down from hospital) in conjunction with community nursing colleagues, as well as thirty intermediate care and rehabilitation beds in the Acorn Unit (an offsite unit) in which the successful candidate can also work. We aim to progress to offering a Hospital @ Home service for care home residents initially in the next twelve months.

The successful candidate is expected to play a role in service development projects and audits in the department and trust wide with a focus on innovating and providing outstanding care for older people across the Barnsley area.

The department has an excellent tradition in medical education including organising and delivering the Royal College of Physicians of London PACES exam. Dr Eltrafi, PhD is a previous college Tutor and a winner of multiple teaching awards. He can offer you support in your teaching career. Additionally, as the Department has strong academic links the successful candidate will be supported to develop their research skills and develop a research interest. Honorary Clinical Senior Lecturer status at the University of Sheffield will be requested for the successful candidate. There is a strong commitment to education and training both in the Trust as a whole, and the post holder will be expected to contribute to this and to the educational supervision of two junior doctors with CME built in to the timetable of this post.

This post includes an on-call commitment, approximately 1 in 15 with lieu time given for weekend working. The department aspires to offer seven day working in the near future, and we expect the successful candidate to take a part., Exceptions can be considered based on candidate circumstances.

The successful candidate is also expected to participate in care of elderly consultant of the week Rota where he/she provides consultant cover for wards referrals, prospective cover for colleagues during absence and SPA time. This currently 1 in 5 weeks

An example timetable is outlined below which may be modified after discussion with the successful candidate. This job description will form the basis of the appointees formal job plan and as such will be subject to annual review and job planning.

This post does include an on-call commitment, approximately 1 in 15 with lieu time given for weekend working. Seven days a week working is currently being reviewed by the Trust and COTE team is expected to provide 7 days consultant cover for Geriatric Medicine
