An exciting opportunity has arisen for a Band 5 (Complex Discharge Facilitator) to Join the D6 (East) nursing senior team. D6East is a 20 bedded Fracture Neck of Femur (NOF) unit offering care to patients who have undergone significant Trauma and the team requires knowledge and skills to optimise patients outcome and recovery. D6 East is a busy, fast paced environment and you will need to be able to work under pressure circumstances.

We are looking for an enthusiastic, highly motivated and dynamic individual who will support both the nursing team and wider MDT with both simple and complex discharges. The successful candidate will work closely with both the ward leadership team and Integrated Discharge Service (IDS) allowing them to be proactive in identifying potential delays and have the skills to generate and pursue solutions. This will not only enhance patient experience by ensuring a safe and efficient discharge from hospital but also support wider flow across the organisation. The successful candidate should have got excellent communication and interpersonal skills and be able to contribute to the development of the ward and work collaboratively with members of the team to provide a high quality and cost-effective service for all patients.

Main duties of the job

  • Support the discharge and Discharge to Assess (D2A) process by providing the background information on referred patients within the ward by liaising with partner colleagues.
  • Support the ward team by ensuring key IT systems are up to date (including bed-view and safe-care).
  • Support the identification and transfers of patients for transfer to community beds.
  • Work in collaboration with the multidisciplinary team (MDT) and the IDS to ensure timely discharge of patients within their caseload when working on the wards.
  • Identify critical delays in the patient care pathway and be pro-active in generating solutions to speed up processes.
  • Deputise in specific (non-clinical) areas for the Senior Sister/Charge Nurse as required to support the smooth operational function of the ward.
  • Be actively involved in any data collection and reviews as indicated.
  • To be visible and available for support in the ward.
  • Support the Senior Sister/Charge Nurse in the management of other complex operational issues as they arise on the ward.
  • To be a focal point for problem-solving and troubleshooting in the daily operations of the ward.

Job description

Job responsibilities

  • To support and co-ordinate discharges through the D2A process by multi-agency coordination and working with patients and families to discharge in a timely manner. Responsibility for a cohort of patients along with the ward nursing team responsibility for co-ordinating their timely discharges.
  • Managing all data and recording of information for cohort of patients.
  • To support on providing advice and support to patients and relatives regarding placements and packages of care. Signposting to services to support discharge.
  • With the ward team, co-ordinate the discharge of patients engaging the IDS and MDT in any concerns or provision of needs.
  • To work autonomously within their scope in, providing comprehensive advice and information to ward staff, patients and relatives, operationally and around discharge.
  • To identify critical delays in the entire diagnostic, treatment and care processes and be proactive in generating solutions that could be out of the norm that will speed up the process eg: escalating to departments and Managers, thinking of services that can stop gap or be purchased in a different manner and bring to the attention of the Senior Sister/Charge Nurse for consideration.
  • To support the Senior Sister in the management of other complex operational issues as they arise on the ward.
  • To assist the ward team in keeping IT flow systems such as Bed-View up to date.
  • To assist the ward areas as directed by the Senior Sister/Charge Nurse when other members of the team are on leave.
  • To attend board rounds and facilitate meetings with staff about discharge and operational issues.
  • To follow the Trusts Discharge Policy and in liaison with Community, Social Services and other healthcare colleagues, to facilitate timely, appropriate and safe discharges of patients to the community.
  • To ensure that confidentiality of information regarding patients is maintained and that both written & electronic documentation is complete, accurate and up to date.
  • To agenda and support multi-disciplinary meetings in response to complex discharges with the goal to resolve any outstanding blocks to a successful discharge.
  • Support the identification of patients who would fit into the parameters of Continuing Health Care and whose needs are high intensity health needs and support referrals.
  • Identify when there is need to resolve disputes or disagreements around operational issues, discharge planning or leaving hospital and to work for a resolution to avoid delayed discharge, being autonomous in this, escalating any issues unable to resolve to the Senior Sister/Charge Nurse.
  • Support on any administration roles and rehab roles where required.
  • To have knowledge and understanding of Power of Attorney and Court Protection and how this impact on patient plans including, power of Receivership Mental Capacity Act Deprivation of Liberty and other legal frameworks.

Key Working Relationships
  • Internally the post holder will develop effective working relationships with the multi-disciplinary team and the IDS working with colleagues from partner organisations including audit social care, continuing health care and community services.

Information and Reporting
  • Accurate data entry IT systems using the correct response times on Bed-View, PAS, Oceano and other available systems.
  • To Participate in audit of the effectiveness of discharge planning within the acute Trust and to collect and analyse relevant data as required.
  • To be up to date with relevant policies and changes in legislation in regards to Health and Social care which may affect the discharge process.
  • To participate on MOFD calls and the tracking of patients.

Staff Training/Education
  • To facilitate and support training relevant to ward teams, including clinical and non-clinical colleagues.
  • To participate in on-going teaching and training and review competencies of staff across professional boundaries, as required by the Senior Sister/Charge Nurse.
  • To attend ward team meetings and share any knowledge, experience or relevant information.

Service Delivery
  • To undertake the defined responsibilities over a seven-day period in response to the needs of the service weekend working may be required, in discussion with the Senior Sister/Charge Nurse and Care Group Matron.
  • To support the Senior Sister/Charge Nurse, Matron and Seniors in supporting safe discharges and operational processes.
  • Accurate records for audit purposes and liaise to review feedback by service users in order to evaluate the effectiveness of the service.
  • To assist the Senior Sister/Charge Nurse in the investigation of complaints in a timely fashion.
  • As part of the ward team, to initiate and develop policies and procedures relevant to the speciality.
  • To support the department and organisation by carrying out any other duties as qualified and able.

Key Result Areas
  • To manage and support a cohort of patients complex discharges and co-ordinate all relevant services to ensure timely, appropriate, and safe discharges.

Management/Leadership
  • The eligible candidate will be responsible for the organisation and management of the ward/department when on duty. Have an overall view of the intensity of the workload and the experience of the staff and act to utilise the nursing skill mix on duty, to ensure the nursing cover achieves a high standard of nursing care.
  • Be proactive in the prevention and management of complaints (formal and those via Patient Advice and Liaison Service) and whenever they occur, investigate and respond in a timely manner. Use any learning to share widely with the MDT to improve discharge process.
  • Comply and promote compliance with Trust policies and guidelines e.g. Health and Safety, Clinical Risk and Infection Control.
  • Take appropriate action to address unexpected changes and situations, informing the Senior Sister/Charge Nurse or other if support is needed and unable to resolve.

Communication and Working Relations
  • Communicate highly sensitive and complex information with empathy ensuring that information is well understood.
  • Maintain excellence in communicating and co-operating with other wards and departments, promoting and maintaining good working relationships within own clinical area and across the organisation, giving accurate information as required and keeping everyone informed of any untoward incidents.
  • Develop expertise in dealing with interpersonal conflict and be responsible for the resolution of any adverse situation/incidents. Comply and promote compliance to Trust policies e.g. Harassment and Bullying.
  • Make sure to be an active member of the clinical nursing team contributing to relevant Trust wide nursing groups and meetings.
  • Represent the Senior Sister/Charge Nurse in their absence at ward level.


Person Specification

Qualifications

Essential

  • Significant specific medical administrative or speciality knowledge gained by education or relevant clinical experience equivalent to degree level.
  • Substantial range of experience with the NHS.
  • Working towards management/leadership qualification.
  • Training in Mental capacity Assessments.


Desirable

  • RGN, NVQ AHP.
  • Evidence of recent study in the Health field.
