Band 6 Tissue Viability Specialist
An exciting opportunity has arisen for you to join our Tissue Viability Team.
The service needs an exceptional, dynamic and self- motivated clinician with wound care expertise with either a Registered Nurse qualification or Podiatric equivalent to provide Tissue Viability Specialist support and advice for patients in both Derbyshire and Derby city.
The successful applicant will be committed to quality improvement of the Tissue Viability Service across the Trust by contributing towards the development and delivery of education and training that will motivate staff to improve practice and to deliver safe and effective care.
You will be a visible and accessible role model, have excellent communication and leadership skills and be committed to improving outcomes for our patients.
Main duties of the job
To promote the Tissue Viability Vision and Strategy, providing support to clinicians and health care workers to prevent and manage wounds by adhering to Tissue Viability Policies and Procedures
Act as a role model for excellence in TV practice and the 'DCHS Way'
To actively engage in delivering and identifying areas for service improvement
To provide Tissue Viability input in the prevention, monitoring and management of wounds, including pressure ulcers, to patients and health care workers in DCHS services, and other settings as required both face to face and by telephone
Liaise with multi-disciplinary agencies to support the ongoing management of patients
To provide support and advice to staff, patients and members of the general public in any matters relating to Tissue Viability.

Leadership
To prompt the Tissue Viability Vision and Strategy, providing support to clinicians and health care workers to prevent and manage wounds by adhering to Tissue Viability Polices and Procedures
Act as a role model for excellence in TV practice and the DCHS Way
Work within and promote DCHS policy and procedures
To actively engage in delivering and identifying areas for service improvement
Deputise for the Tissue Viability Matron and other Clinical Team Lead as required
Patient Care
To provide Tissue Viability input in the prevention, monitoring and management of wounds, including pressure ulcers, to patients and health care workers in DCHS services, and other settings as required both face to face and by telephone
To provide a pathway of support for patients with Tissue Viability requirements moving between different health care and community settings
Liaise with multi-disciplinary agencies to support the ongoing management of patients
To provide support and advice to staff, patients and members of the general public in any matters relating to Tissue Viability.
To support and conduct Root Cause Analysis (RCA, serious incidence or coroners investigations relating to Tissue Viability
Strategy and Service Development
To contribute to Tissue Viability policy and guidelines, including pressure ulcer prevention across DCHS.
To work in partnership with other Tissue Viability teams across the Derbyshire Health Community
Develop communication and working practices within Derbyshire health Community
to develop and deliver educational sessions
To collect, analyse and provide relevant data and reports for the Health community with regard to Tissue Viability issues
Financial Resources/Management
Identify and support the implementation of initiatives to achieve cost improvements within Tissue Viability related work streams
Identify and support income generation opportunities
Ensure appropriate use of resources within the Tissue Viability service
Information Resources/Information Systems
Maintain electronic records of advice given on patient care, compliance of care with policy and patient outcomes
Autonomy/Scope within Role
Manage own work load under the support and supervision of the Tissue Viability Clinical Lead
Support an urgent/immediate response to an emergency situation. This will require flexibility and the need to prioritise work accordingly.
