Job summary

The Diabetes Team at SaTH are delighted to invite applications from an experienced DSN with Diabetic Medical Device knowledge and skills to take forward and build on the existing service in place. This is a new post and is fixed term for 12 months.

You will be self motivated and passionate about delivering patient focussed care and an experienced Diabetic Specialist Nurse.

You will work closely with the Nurse Lead Clinical Nurse Specialist and will deputise when required with leading and managing the team.

You will need engagement with all Corporate and Divisional objectives, through strong visible leadership and role modelling you will ensure patients are treated with compassion, dignity and respect.

You will provide professional and clinical leadership within the Diabetes devices service and lead on specific Professional and Service projects as required. This is a unique opportunity to lead the Diabetic Devices service.

You will work closely with colleagues in the ICB on our Diabetes System Transformation.

Establishing relationships with MDT and key stakeholders within and external to the Trust will be key to the service delivery. You will have experience preparing reports and putting action plans in place to support with recovery of any key standards.

Main duties of the job

As a Registered Nurse you will be working at an advanced level undertaking tasks that have been previously undertaken by medical and/or technical staff. As a Practitioner you will practice autonomously and also within a defined multi-disciplinary team to provide advice and support to patients, carers, and healthcare professionals on the Diabetes Device Service (insulin pumps and Continuous Glucose monitoring), across both secondary and primary care and in accordance with local guidelines and policies.

You will formulate clinical decisions and treatment plans within local guidelines /policy to manage acute and chronic illness and will respond to actual and potential health problems appropriately. You will undertake advanced practice procedures and tasks which will provide holistic nursing care and therapeutic nursing interventions.
  • To work collaboratively with the Nurse Lead Clinical Nurse Specialist for Diabetes and Endocrine and to deputise where required.
  • Participate and support teams in the delivery of compassionate, safe and effective care, maximising efficiencies, ensuring optimal performance and productivity by continually monitoring standards of care and taking remedial action to address any concern
  • Responsible for identifying any patient safety concerns and take appropriate action to mitigate against any risk to patient care and ensure the team members monitor and mitigate risks via their individual risk registers.


About us

You will have flexibility to work cross site between our Shrewsbury and Telford Hospitals.

This is a fixed term post for 12 months.

Qualifications

Essential

  • Non-medical prescribing or working towards within 2 academic years of appointment
  • GuidanceScore MSc in Health, Quality or leadership related or working towards within 2 academic years of appointment
  • Management Qualification (ISM) or relevant management experience
  • NMC Registered
  • RN Diploma or Degree


Experience

  • Extensive experience at senior level/ leadership role or ability to demonstrate


Knowledge

  • Good knowledge of national and local targets, NICE Guidelines and policies in relation to diabetes care
