North Staffordshire Combined Healthcare NHS Trust are looking for a compassionate, enthusiastic and motivated Band 5 Nurse, RMN, RGN, RLDN, to join our integrated Substance Misuse in-reach team at Stoke Heath Prison. The role is a band 5, 37.5 hours per week and is ideal for those wanting to take the next step in career progression.

The post holder will be working within the in-Reach team delivering interventions with those affected by substance misuse, working alongside the Secondary Mental Health team providing psychological interventions to those requiring support.

Experience and previous knowledge of substance misuse would be beneficial, but not essential. We will provide training, support and regular 1:1s to ensure you have the tools you need to succeed in this role.

Main duties of the job

Under the supervision and direct management of the Integrated Substance Misuse and Mental Health Manager, you will contribute to the provision of high quality substance misuse services which provides recovery focused clinical treatment for the prison population within Stoke Heath HMP.

To work within a strengths-based, recovery-orientated, change and outcomes focused approach which promotes service users as responsible co-producers of health, well-being and recovery.

To work collaboratively with other services to provide interventions to support and motivate service users and meet their needs through all stages of their individual recovery journey.

Flexibly support the out of hours service delivery during specific service demand.

To act as named nurse / care coordinator for a specific group of prisoners and provide care under the supervision of the clinical service manager. To deliver specific psychosocial interventions to this group of prisoners and liaise with other professionals regarding progress and outcomes.

To ensure that practice remains abreast of new developments and to develop and maintain standards and implement change in respect of this.

To liaise with other professionals and agencies both statutory and non-statutory and to provide a link between the prison and community

Job responsibilities

To act as named nurse / care coordinator for an identified group of service users within the clinical caseload providing recovery focused, evidence based clinical practice to facilitate effective treatment decisions.

Engaging with the prisoners who have substance misuse problems, to address their individual issues working alongside the medical and psychosocial therapies teams to improve health and social functioning.

Initiate service users safely onto clinical treatment regimes ensuring that clinical protocols are constantly reviewed to promote best evidenced clinical practice linking to National Guidelines and Trust policies and procedures.

Undertake assessments and having the responsibility for documenting the plans of care plans ensuring they remain current and that standards of record keeping are adhered to. To take an active role in auditing and maintaining standards of record keeping for this group of prisoners.
