An exciting opportunity for an experienced ACP or Junior Doctor to shape and develop work within the Joined-Up Care Derbyshire System with a Personalised Care focus with a 12-month fixed term contract for up to 4 days per week for ACPs or 2 days per week for Junior Doctors.

Increasing amounts of evidence are illustrating the impact which Personalised Care can have for both patients and clinicians and there are a significant number of projects underway within Joined-Up Care Derbyshire to look to implement these principles within service design and transformation to maximise patient experience and outcomes.

We are excited to be able to offer up to 2 fellowships to work with projects identified within the Derbyshire System: working with University Hospitals Derby and Burton with their team on the vital and challenging transition from paediatric to adult services or working within Primary Care in Derby City developing their service to improve care offered to those with complex multi-morbidity. The range of projects gives flexibility for the successful candidate to follow their preference and which best meets their interest and skillset.

Main duties of the job

Initial steps will be to define and scope the project, identify the aims and current situation, engaging with key system partners to do this effectively.

Leadership and change management skills will be needed to develop and then adhere to timelines and deliver the personalised care focussed project.

As the project progresses it will be important to develop resources and learning as appropriate to support the change required for the project and also to develop outcome measures for success.

Evaluation is the vital element to the fellowship to ensure that feedback from all stakeholders (providers, services, health professionals and of course service users) in order to determine the impact of the project and further developments and learning which may be required.

The post holder will not be expected to provide direct patient care as part of this role. However, it is vital that their own clinical expertise is maintained in order to maintain capability, authority and credibility.

• Through collaborative joint leadership, develop and deliver personalised care -centred project within the Derbyshire Integrated Care System.
• Responsibility for creation of learning resources which can be shared across the area in order to support the project as needed and appropriate.
• Engage with key system stakeholders to ensure that active participation in the development of the project is achieved and input into development is received from a variety of professional backgrounds and view points.
• Work collaboratively to identify and overcome difficulties with engagement in the process
• Critically appraise project outputs to ensure that they align with both local provider, ICB and national priorities adapting the offer as necessary.
• Actively seek and analyse feedback from professionals, patients and key stakeholders and adapt the offer in light of feedback received to maximise the potential future successful outcomes from the project.
• Ensure that the provision is based upon current research and best practice.
• Effectively manage own time, workload and resources to ensure maximal performance within the role alongside personal wellbeing.
• Actively collaborate with system partners, professional teams and patient representatives to ensure that the offer meets their needs

Education & Training

Essential

• Registered with GMC with license to practice for medical applicants
• Junior Doctor - MBBS (or equivalent)

Desirable

• Teaching or mentorship qualification

Experience & Knowledge

• At least 12 months post-qualification as a Junior Doctor
• Experience of mentoring, supervising and teaching
• Involvement in service improvement and implementation of best practice
