We are looking for Mental Health Social worker to join our responsive Community Crisis Team.
Working in Portsmouth offers a diverse inner-city client base where we provide intensive treatment and interventions to individuals who present with a range of mental health needs as a 24 hour service. This role is a core hours post that does not require any night time working. We need you to travel across the city to visit our patients, either by bike or bus, car or train! We also have pool cars which can be used by people with full UK driving licences and if you want to use your own, you can claim mileage!
Our aim is to prevent hospital admission and to be the access point for urgent mental health care to adults across Portsmouth.

Main duties of the job
To work as an active member of the integrated multi-disciplinary team. This role is to work core business hours (9-5, Monday -- Friday) but supporting a team providing an acute, mobile 24-hour community-based Crisis Resolution Home Treatment Service (CRHT) as an alternative to hospital admission.
To support the team to manage a caseload of clients with complex social care needs providing interventions using evidence based, client centred principles to aid recovery and avoid admission to psychiatric hospital. To ensure that the service provided is high quality, clinically effective and cost effective.
When required, to provide rapid assessment in a range of settings and help plan and deliver effective, evidence-based home treatment in collaboration with service users, carers and professionals from other established services. Strong emphasis is placed on assisting service users to learn from the crisis.

- Social Work England registered Social worker, degree level or equivalent
Experience

- Previous experience of working with users and carers using a recovery approach.
- Demonstrate understanding and experience of using the Care Programme Approach, risk assessment and management
- Ability to use information technology
- Ability to work flexible hours to meet the needs of the service.
- Knowledge of liaison role between the CRHT/residential/community teams.
- Show an understanding of caseload management.
