Applications are invited for Clinical Fellow Doctor in Obstetrics and Gynaecology. The post will be for general clinical and administrative responsibilities in Obstetrics and Gynaecology at Middle Grade Level.

This post is for a two year fixed term, one year at Colchester and one year at Ipswich in rotation confirmed on appointment.

These includes labour ward, antenatal clinic, ambulatory obstetrics, elective caesarean section lists and maternity inpatient care.

It also includes gynaecology outpatients, rapid access clinics including gynaecology ultrasound scanning skills, out-patient hysteroscopy, colposcopy, acute /emergency gynaecology, gynaecology theatre including robotic theatre experience and inpatient care. There will be an associated supportive professional activity for aspects of continued professional development and appraisal/revalidation.

Main duties of the job

The Clinical Fellow doctor will have a responsibility of supporting the Consultant Obstetrics and Gynaecology team. They will work at the Middle Grade level in terms of clinical responsibility. They will have responsibilities in clinical and administrative roles within Obstetrics and Gynaecology. Additionally, the post holders will take part in the on-call/out of hours rota (1:8/1:9) in line with the requirements of the department, which will form part of the job plan. Responsibilities will also include the supervision of junior staff and involvement in a regular teaching programme as well as participating in weekly meetings and monthly audit meetings.

There will be support for CPD and appraisal within the job plan in line with current NHS contract.

Opportunity may arise for the Clinical Fellow doctor in developing a specialist interest or skill, dependent on the requirement of the applicant and the needs of the department.

Job description

Job responsibilities

Clinical:
  • On call and Emergency cover
  • Out of hours (on call) there is a 1:10/1:11 rota for middle grade doctors. Handover is 08:30 and 20:30 hours. For out of hours (20:30 - 09:00 hours) there is a resident ST grade or Speciality Doctor covering Delivery Suite, Maternity Inpatients, as well as Gynaecology Emergencies and Inpatients. This role is supported by a junior grade doctor resident, and a Consultant on call (non-resident).
  • During normal working hours 08:00-17:30, the emergency duties are shared between a middle grade for Delivery Suite and a Middle Grade for Wards and Gynaecology Emergencies.
  • From 17:30 the Gynaecology Middle Grade will handover care to the Obstetric/Labour Ward Registrar.
  • A Consultant is resident in the hospital for Delivery Suite and Ward Round from 08:30 20:30 hours
  • The on call/emergency cover role takes precedence over all other clinical work and you are expected to remain within the hospital grounds when you are the duty registrar.
  • At changeover time there should be a face to face handover of clinical cases and the bleep on the Delivery Suite.

Obstetrics:
  • Ward Rounds on the Maternity Ward both antenatal and postnatal patients
  • Antenatal Clinic
  • Maternity Triage and Antenatal Assessment Unit
  • Delivery Suite Management
  • Elective Caesarean Section Lists
  • Accept referrals and review Obstetric patients admitted to other parts of the hospital

Gynaecology:
  • Ward Rounds and assessment of patients on the Gynaecology Ward for both acute admissions and elective surgical cases
  • Gynaecology Theatre Inpatient and Day case Lists (level of supervision as appropriate to surgical cases and experience of the doctor), Robotic and Laparoscopic theatre assistants opportunities can be discussed to train as assistant. And to attain Laparoscopic competencies
  • Gynaecology Outpatients
  • Emergency Gynaecology assessment and initial management of acute/emergency attendances both on Gynaecology Ward as direct referrals and in Accident and Emergency Department
  • Accept referrals and review Gynaecology patients admitted to other parts of the hospital
  • Rapid Access Clinic Gynaecology Ultrasound scanning, Out-patient hysteroscopy
  • Colposcopy

Supervision and Support of Colleagues The Department is supported by a number of junior grade doctors at various levels - ST1/2 in O& G, GPST, and foundation year doctors. The role of the middle grade doctors as their immediate senior, is to provide support and supervision in caring for patients in both Obstetrics and Gynaecology. With the transition of ST2 to ST3 in Obstetrics and Gynaecology Speciality training, the department runs a system of on call resident middle grade support for the ST3s for their first 6 months on the middle grade rota. The speciality grade doctors have previously supported the junior grades in this transition by being resident on call while they do their nights.
