Rapid Response Advanced Practitioner

Croydon Health Services are in a new Alliance Partnership with other health and social care providers in Croydon and the LIFE service is one of our new integrated service models. This means bringing together health and social care teams who work to support people to remain independent and healthy in particular to prevent admissions to hospital and shorten hospital stays.
The Rapid Response Service is an admission avoidance service available 24 hours a day, 7 days a week.
Main duties of the job
The post holder will be an autonomous practitioner who will be expected to take a lead in the advanced assessment, management and treatment of patients over 18 years of age referred to thecrisis service. As an autonomous practitioner you will provide comprehensive healthcare sensitive to the needs ofthe population, including advanced clinical assessments, history taking, diagnostics, treatment planning, decision making, non-medical prescribing and evaluation. Working as part of a multidisciplinary team and across the primary, secondary and social care interfaces you will be responsible for co-ordinating the provision of specialist interventions to meet the urgent health care needs of people at risk of hospitalisation or those requiring a facilitated timely discharge from ED, SDEC and ACE and supporting a more intensive level of care in the intermediate care environment.The post holder will provide specialised input as an independent and interdependent practitioner developing and leading relationships with members of the multi-professional team allowing for consultation and collaboration in a variety of health care settings including the community, primary care and the acute hospital.As a senior clinical decision maker, you will aim to prevent unnecessary acute hospital admissions and keep patients in their own homes for longer and prevent premature admissions into long termcare facilities. EDUCATION AND QUALIFICATIONS
Essential
- MSC in Advanced Clinical Practice Or In process of completing MSC in Advanced Clinical Practice but has already completed Independent non-medical prescriber (V300) and has at least 2yrs practice with wide scope of practice
