Job summary

We are looking for an enthusiastic Senior Clinical Pharmacist to work closely with healthcare teams in Great West Road Primary Care Network (PCN) - to deliver our pharmacy vision, delivering excellent pharmacy-led services for the people in this PCN's neighbourhood.

Experience in General Practice is desirable. The Senior PCN pharmacist will be expected to contribute to the leadership of medicines-related issues for Great West Road PCN, and to provide leadership and mentoring to any clinical pharmacists, pharmacy technicians and prescribing support staff employed across the PCN.

We are most interested in candidates with:
  • Up-to-date clinical knowledge who understand the nature of primary care prescribing, medication review, and strategies for improving prescribing. A commitment to addressing health inequalities and proactively reaching people from all communities
  • The ability to recognise their personal limitations and boundaries, to understand those of their colleagues, and refer to other colleagues when necessary
  • The ability to thrive when working in a devolved, flexible, autonomous environment
  • You will be practicing at an senior level, have excellent written and verbal communication skills, experience of influencing others, conducting and managing face to face and remote clinical caseloads in primary care.


Main duties of the job

You will work as part of a multidisciplinary team to: o Undertake a patient facing role in managing long-term conditions; working directly with patients to assess and treat conditions, as well as promote self-careo Deal with management of medicines on transfer of care: discharge letters, treatment advice notes and medicines reconciliation,o Develop and run processes for repeat prescription re-authorisation,o Perform structured medication reviews,o Work with practices enabling them to develop and apply systems for safer prescribing,o Develop good relationships between local community pharmacies, and the PCN pharmacy team,o Develop and deliver medicines-related training for GP and ACP prescribers in the PCN, and education events for patients.

Your development

We will support you to develop your role, including enrolment on the General Practice Pharmacist Training Pathway from CPPE (unless an equivalent qualification is already held and exemption is agreed by CPPE) and in becoming an independent prescriber if the qualification is not already held. You will have line-management from the PCN, plus a GP prescribing mentor. Peer support is vital for this role and will be provided by pharmacy colleagues within PCNs across Hounslow and Hounslow PCN Support team.

Qualifications

Essential

  • Mandatory registration with General Pharmaceutical Council (GPhC)
  • Post-graduate diploma level or equivalent
  • Extensive knowledge of medicines optimisation,


Desirable

  • Independent prescribing


Skills and Abilities

Essential

  • An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
  • Recognises priorities when problem solving, identifies deviations from the normal pattern, is able to refer to seniors or GPs
  • Work effectively independently and as a team player


Desirable

  • Management experience either clinical or people


Knowledge

Essential

  • In depth therapeutic and clinical knowledge, understands the principles of evidence-based healthcare
  • Knowledge of IT systems: word processing skills, emails and the internet to create simple plans and reports


Desirable

  • Demonstrates accountability for delivering professional expertise
