We are excited to have created a novel Tier 3 (middle grade) Senior Clinical Fellow (SCF) post in 2022 and are now looking for a new SCF to join our dynamic and friendly Emergency Medicine team, as part of a large investment expanding our medical workforce. You will pursue a special interest and also work clinically whilst working in Nottingham University Hospitals (NUH) Emergency Department . NUH is the East Midlands Major Trauma Centre.

The Senior Clinical Fellowship in Point of Care Ultrasound and Regional Anaesthesia would be an ideal post for someone looking to improve their skills and knowledge in medical uses of ultrasound. In this post, you will spend 50% of your time being exposed to a variety of supervised learning opportunities in different settings. The remaining 50% will be spent working clinically out of hours (including nights, weekends and evenings) to gain clinical experience in a busy Emergency Department.

We can offer you an excellent training opportunity to enhance postgraduate clinical, leadership and management experience in adult and paediatric emergency medicine in a large, research active university teaching hospital and major trauma centre . You will develop expertise in Ultrasound and Regional Anaesthesia during the course of the year.

Main duties of the job

This post is full time in Emergency Medicine, in which:

• 50% of your time will be spent pursuing your interest in POCUS and Regional Anaesthesia in a variety of settings and supporting our departmental education team, there may be the opportunity to undertake a PGCert in Medical Ultrasound at the University of Derby.
• 50% will be clinical, on the Tier 3+ (ST3+ level) registrar rota providing cover in all the different areas of our ED (Majors Unit, Resuscitation Area, Urgent Treatment Unit, Paediatric Emergency Department).

The responsibilities and duties of the Emergency Department element of the post include:

• The assessment and provision of treatment to patients attending ED and liaising with appropriate specialties for ongoing management
• Acting as resuscitation team leader for seriously ill or injured patients, including children presenting to ED.
• Liaising with the shop floor consultant in charge for clinical and administrative advice as appropriate.
• The supervision and training of junior doctors, Advanced Practitioners, Physicians Associates, Urgent Care Practitioners and Emergency Nurse Practitioners .

You will be expected to take a leadership role in our Radiology and Anaesthesia Interface Collaboratives to improve patient pathways in these areas, as well as supporting quality improvement, research and educational innovations.

Point of Care Ultrasound and Regional Anaesthesia

NUH has a wealth of experienced clinicians with a proven record in the education and training of specialty trainees. You will spend time in our radiology department and with various specialties, including Anaesthesia, Cardiology and Respiratory Medicine to support their learning and clinical application of skills.

Our Emergency Department

Our forward thinking, research active Emergency Department is one of the busiest in Europe with 179,000 new attendances annually. The Children's ED sees 44,000 children and young people and has recently benefitted from a transformation to expand the department with major investment.

The Department has 24 hour Consultant cover, and the recent workforce investment will support an increase across the entirety of our clinical establishment. The appointee will join our team of 40 Tier 4 (middle grade) clinicians, (Higher Specialist Trainees, CESRs and senior Advanced Clinical Practitioners).

The nine bayed Resuscitation Area includes two adult and one paediatric major trauma bays, and the Majors Area has 30 monitored bays. The Urgent Treatment Unit (UTU) is a co-located area where ambulatory illness and injury patients are assessed and treated by primary care and ED clinicians. The expansion and rebuild of our new paediatric emergency department will include a new purpose built paediatric resuscitation area.

The ED also runs a Clinical Decisions Unit alongside the main department.

Job responsibilities

The post is open to all applicants with a demonstrable interest in POCUS and Regional Anaesthesia, who meet the essential criteria detailed in the person specification and wish to gain further experience in the management of a wide range of medical and surgical conditions presenting to all areas of the Emergency Department.

You will aim to develop expertise in POCUS and regional anaesthesia based on the RCEM curriculum objectives, with objectives and programme of learning to be agreed following an appraisal of current skills and experience, and determination of learning needs.

If successful, you will be allocated clinical supervisors for Emergency Medicine and Ultrasound who will support the individual to develop their clinical and non-clinical skills. You will maintain a logbook of experience, complete appropriate assessments and participate in the wide range of non-clinical opportunities available.

This post would suit an experienced Emergency Medicine clinican, ACP or Post CCT clinician wishing to develop their skills and knowledge of Improvement in the Emergency Medicine setting.

The responsibilities and duties of the Emergency Department element of the post include:

• The assessment and provision of treatment to patients attending ED and liaising with appropriate specialties for ongoing management
• Acting as resuscitation team leader for seriously ill or injured patients, including children presenting to ED.
• Liaising with the shop floor consultant in charge for clinical and administrative advice as appropriate.
• Working with the nurse in charge and the wider multi-professional team to ensure effective running of the department, supporting the delivery of safe, timely, quality care, in line with the National Emergency Care Standards
• Contributing to the provision of an effective emergency medicine service in collaboration with colleagues in the multi-professional team.
• The supervision and training of junior doctors, Advanced Practitioners, Physicians Associates, Urgent Care Practitioners and Emergency Nurse Practitioners .
• Attendance and presentation at departmental and Trust Governance meeting

Full registration with the GMC or relevant registered body is essential. All applicants should be competent to work to Tier 3+ (middle grade) level with at least six months experience in Emergency Medicine at registrar/Senior ACP level. They would ideally have at least 3 months experience in anaesthesia (including completion of Initial Anaesthetic Competency certificate) and experience of critical care is desirable. Prior experience of working in the NHS or a comparable healthcare system is essential

Training and Qualifications

Essential

• MB ChB or equivalent. OR MSc and ACP qualification
• GMC registration with licence to practise or equivalent OR if ACP, registered with relevant professional body
• Evidence of completion of life support courses: ATLS or equivalent; ALS or equivalent; APLS or equivalent.
• Evidence of progression towards Level 1 Ultrasound Competency

Desirable

• Full FRCEM or Certificate of Completion of Training (CCT)
• Level 1 Ultrasound competency

Experience

Essential

• 6 months previous Emergency Medicine experience at ST3+ level (or equivalent) OR as qualified ACP in the NHS or a comparable healthcare system.
• Completion of Foundation training plus 2 years of post-Foundation training in acute specialties (Anaesthesia, Acute Medicine, Emergency Medicine or Intensive Care Medicine) or equivalent OR significant experience working as a qualified ACP

Desirable

• 3 months anaesthetic experience (including certificate of Initial anaesthetic Competency).
