Job summary

The post holder will be required to work within a multi-skilled team supporting the assessment and treatment of children who have a variety of continence issues, predominantly within Derby City. This team is part of a sub- contract team, which works as part of the Derbyshire-wide Children's Continence Service lead by Chesterfield Royal Hospital.

The post holder is responsible for managing a caseload of children & young people accepted for assessment and treatment for a range of continence difficulties. The role will involve supporting the Senior Nurse in the day to day running of the service in the Derby City area, including the supervision of other team members and the triaging of referrals.

Main duties of the job

  • Communicate effectively with children of all ages who may have a range of physical and/or psychological needs and their parents/carers.
  • Adapt communication skills to take account of communication needs and differences in order to plan, deliver and evaluate care.
  • Work closely and effectively with the other team members of the Children's Continence team, ensuring the Senior Nurse has full clinical oversight of the work being undertaken and that which remains on waiting lists.
  • Communicate effectively with both internal and external services to best meet the needs of the child/young person.
  • Make appropriate referrals to internal and external services as required.
  • To be able to identify cases where expedition is necessary and to be able to action this accordingly with clear process and procedure rationale.
  • Undertake routine management and organisation of their own time, and others they manage/ supervise.
  • Manage a caseload of children from assessment through to discharge.
  • Plan work and report to Senior Nurse.
  • Gather and analyse a range of information and have the ability to draw conclusions and develop care plans with families.
  • Clarify and resolve any discrepancies or difficulties during the assessment and treatment process.
  • Document key points of agreement, including specific referrals, interventions and recommendations.
  • To comply with safeguarding children procedures and training requirements.

You will work as part of the Derbyshire Children's Continence Team which has three team (Derby City, South and North Derbyshire). This post will be based in Derby City in the Children's Division of Derbyshire Healthcare NHS Foundation Trust. We have a wide range of other children's services 0-19s Integrated Public Health, Community Paediatricians, ADHD Nurses, Intellectual Disability Nurses, School based Nurses in Special schools, Occupational Therapy and Physiotherapy and CAMHS. Therefore, you are in the centre of children's community health services in Derby City.

Knowledge & Experience
  • 1st Level Registered Nurse with current relevant registration.
  • Post registration experience of working with children in a community setting.
  • Knowledge and experience of working with families and children who have continence issues and the associated emotional and/or behavioural difficulties and associated comorbidities.
  • Experience of managing waiting times, and dealing with associated enquires.
  • Experience of signposting families to interim services that may be more appropriate to support the needs arising.
  • Experience of analysing data and presenting this in a useful way to project and forecast information.
  • Excellent communication and organisational skills.
  • Knowledge and experience of assessment processes.
  • Knowledge of NICE Guidance in relation to Childrens Continence.
  • Knowledge of and experience of working within the SEND code of practice.
  • Sound knowledge of children and adult safeguarding procedures.
  • Experience of multi agency and multi professional working.
  • Experience of teaching parents, other professionals and team members.

Skills
  • Ability to gather and analyse a range of data sources and be able to draw conclusions and present the information in a succinct report template, analysing the information to compile an outcome.
  • Ability to oversee incoming referrals ensuring information is recorded correctly and prioritised accordingly.
  • Ability to actively manage own time and manage that of others you supervise effectively and meet deadlines.
  • Ability to follow up on outstanding or delayed information knowing when to use assertive approaches.
  • Ability to work with a solution focused approach in order to effectively prioritise or problem solve situations.
  • To have a sound understanding of the components of a comprehensive and holistic assessment of a child or young person with possible continence difficulties.
  • The ability to support and promote understanding of the needs of children with continence difficulties and co- morbidities to other professionals, children, young people and their families.
  • Ability to identify potentially risky/vulnerable situations and take necessary action to expedite assessment and engage the required support working in conjunction with the child and family where possible.
  • To actively implement the Trust Values in daily clinical practice and evaluate the effectiveness of this through supervision, audit and appraisal.
  • Ability to use resources effectively in a cost-effective way.
  • Ability to demonstrate effective IT skills including record keeping, waiting lists, report writing, spread sheets and electronic diary through the use of national and organisational systems.
  • Work with the Senior Nurse to ensure own and other team members clinical skills and competences are maintain.
  • Ability to reflect on personal practice.
