Band 6 OT or Physio Postural Management Team. Band 5 applicants will be considered as a Development Post to Band 6
The post holder will be a key member of the Postural Management Team, which are are integrated with Care Home Support Team (CHST). The Care Home Support team includes a, Pharmacist, Registered General Nurses, Registered Mental Health Nurses, Palliative Care Nurse and Therapists.Based in Reading you will be a team player within the Berkshire West locality and expected to work collaboratively with staff from all disciplines whilst maintaining high professional standards.
The proactive branch works 5 days a week, Monday to Friday, 9-5, however the post holder has some flexibility depending on the needs of the care homes. This is a full time 37.5 hours a week post .

Main duties of the job
o To independently manage a caseload of complex cases of all ages relating to mobility, pressure and postural care and often in combination with communication, challenging, cognitive and/or perceptual difficulties.
o Formulate individualised action plans based on the assessment findings and clinical reasoning prescribe appropriate specialist equipment using options from a range of equipment.
o To work as part of the MDT to meet key performance indicators as set by the ICB e.g. falls prevention, reduction in pressure ulcers through provision of postural management equipment for residents with contractures.
o Work as a senior practitioner and be professionally and legally responsible for all aspects of your own work.
o Deliver a pro-active training/educating advisory service that aims to improve the health, well-being and independence of all care home residents.
Champion care home residents' rights, for example dignity, equality, diversity, ethnicity and culture, and ensure services are delivered in a manner that is appropriate, stimulating, relevant and respectful recognising choice and the decision-making capacity of the resident
