Community Urology Nurse Band 5

We are looking for Registered Nurses with an interest and passion in Urology to support the delivery of high quality care to our patients. This will initially be focusing on our patients with an indwelling catheter, and those patients awaiting trial without catheter. Once embedded into the team it will then involve moving onto other pathways within Urology and continence such as bowel and bladder assessment/reassessment, and give opportunities to develop clinical skills and knowledge. These are new roles created to reside within the Placed Based teams working across 5 days to support this group of patients. Main duties of the job

To work within the Place Based Teams, supported by the Continence Team, to provide a comprehensive nursing service to patients with a catheter. To be an integral part of Coventry and Warwickshire Partnership Trust Out of Hospital Service.

The role to include holistic initial catheter assessments, pre-twoc (trial without catheter) assessments and twocs, Teaching ISC (intermittent self-catheterisation) to patients, undertaking bladder scanning, managing patients with complex catheter problems, supporting colleagues to gain competence around indwelling catheterisation.

To work in collaboration with all 'Place based community nursing teams', the acute services, social services, and Allied Health professionals with an interest or involvement in Catheter and Continence management, to ensure quality service delivery, using local and national guidelines.

To identify the required support for patients and their carers/family in relation to catheter care provision, therefore the post holder will require the skills to maintain, restore, and promote health, empowering individuals to gain their optimum independence.

To provide effective evidence-based care to patients on a defined case load.

To develop and maintain joint working practices with CWPT internal and external partner provider agencies.

To provide advice and guidance to other members of the multi-disciplinary team, patients and carers.

Qualifications

Essential

  • RGN NMC registered First level Nurse


Knowledge and skills

Essential

  • Ability to carry out comprehensive risk assessments and risk management plan
  • Evidence of good clinical skills and an ability/willingness to share and teach others
  • Knowledge of Continence Assessment / Catheterisation and catheter care or be willing to develop
  • Knowledge of Long term Conditions or be willing to develop
  • Ability to assess a wide variety of clinical health needs
  • Plan, implement and evaluate programmes of care


Experience

Essential

  • Experience of working in a multi-disciplinary team and able to demonstrate a positive attitude towards it.


Other

Essential

  • Car owner/driver with insurance cover to include business use.
