We are recruiting Band 5 Staff Nurses into the Bradford and Airedale Palliative Care Support Teams. We support patients who are end of life across Bradford in the community to provide high quality care to patients at the end of life. Working closely with community nursing teams, Fast track teams and palliative care teams, you will support patients to prevent unnecessary admissions to hospital, and support them in their preferred place of care
In addition, you will work alongside other key members of the primary health care team, other health and social care professionals, and carers, to develop and implement personalised care plans for patients at the end of lifeThis is a great opportunity to gain experience & one in which you will be supported by your colleagues, & Service Manager.

Main duties of the job
To deliver high quality time limited programmes of care for patients who are at the End of Life (EoL). To support the development of the palliative care team as a whole and provide a consistently high standard of care to patients and their carers both paid and unpaid.
