An exciting opportunity has arisen for a part-time Community Triage and Response Nurse to join our Integrated Teams in Barking and Dagenham as part of a job share.

We are seeking innovative, caring and dedicated registered Nurse to compliment the Barking & Dagenham Community Health and Social Careteam. Our community Nurses support the delivery of 24hr holistic health care to support our strategic vision of the 'Best care by the best people'

The post holder will be a Registered Nurse (RN12-NMC) responsible for patients allocated to them and will assess, manage, plan and deliver care, including being professionally and legally accountable.

The post holder will support their peers and team leader and be an effective team member whilst working on their own initiative. They will have a commitment to collaborative working and actively support and liaise with other health and social care professionals and agencies.

Main duties of the job

The post holder will work within the Community Nursing Team, geographically aligned to GP practices and be responsible for clinical triage and unplanned nursing care, providing clinical review of patients and engaging with health and care professionals to enable effective patient flow and referral management. They will work as part of the integrated primary/community health care team, working within and across professional and organisational boundaries. Liaising with GPs, other professionals and statutory and voluntary agencies he/she will deliver individualised care to patients.

You will be travelling around our borough to deliver community based care to patients in their own homes, therefore, a full UK driving licence and access to a car for work purposes is essential.

About us

Starting with NELFTNELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Probationary PeriodThis post will be subject to a probationary period. Qualifications

Essential

  • Current NMC/ RN or any other 1st level Nursing, RN12 Adult Qualification
  • Evidence of post-registration education and training


Desirable

  • Mentor qualification
  • Relevant post-registration course e.g. wound management, diabetes, leg ulcer care, continence care and assessment
  • Nurse Prescriber (V150)


Knowledge

Essential

  • An awareness of NHS plan, NSF and clinical governance priorities
  • The content and application in practice of the NMC Code of Professional Conduct
  • Able to initiate patient assessment and care planning with the support of a senior colleague
  • Able to work with patients and carers as partners in care


Experience

Essential

  • Experience of working with professionals from other disciplines necessary
  • Experience of working as a member of a team but also to work autonomously
  • Experience of providing packages of care, which involve other agencies
  • Reflective Practitioner


Desirable

  • Community Nursing Experience
  • Experience of teaching and assessing students and other members of staff
  • Experience of acting up in the absence of team leader in caseload and team management
