Job summary

An exciting opportunity has arisen to employ a Mental Health Practitioner for a 12-month fixed term or secondment opportunity. This role will focus on the delivery of population based mental health care for the community of Wyre Forest. You will be working alongside a team of highly specialist practitioners and have direct relationships with the professionals within the local Primary Care Networks (PCN). If you are seeking a role that will encompass new ways of working that will be the future delivery of community mental health care in England, then this is an opportunity you should not miss. Consideration will be made for flexible working and there may be some opportunities for staff members to work from home where clinically possible. Although this is a secondment/fixed term opportunity, there may be opportunity to apply for a permanent position in the team depending on future circumstances.

Main duties of the job

The main duties of a Mental Health Community Practitioner within our team includes (but not limited to):
  • Completing assessments on patients referred within the Wyre Forest locality. Referrals would typically come from GPs, however, may also come from internally within the trust or other agencies.
  • Holding a caseload of patients with a range of mental health illnesses and providing treatment to these individuals using evidence-based interventions as appropriate.
  • To develop care plans with patients and the MDT, to the utilise these care plans to support them in recovery.
  • To be able to risk assess and risk manage patients in response to their current needs.
  • To take part in the duty system and act appropriately when dealing with patients who are experiencing difficulties in the absence of their allocated worker.
  • Receive clinical and managerial supervision regularly, and to also supervise and support junior members within the team.

Additional Criteria

Essential

  • Will be required to complete breakaway / de-escalation training
  • To participate in regular locality duty system if required
  • Able to meet the travel requirements of the post


Experience

Essential

  • Experience of working in a Mental Health setting
  • Assessment of people presenting with mental health problems
  • Assessment and management of clinical risk
  • Working within a multi-disciplinary team
  • Managing own case load


Desirable

  • Working in a primary care setting
  • Working in collaboration with VCSE service
  • Experience of Care Coordination
  • Training/developing staff and / or patients
  • Experience of delivering evidence based psychosocial interventions


Qualifications

Essential

  • Relevant professional qualification relating to Health or Social Care e.g. in social work, nursing, OT with current professional registration
  • RMN, BSc/DipSW, BSc OT
  • Recent evidence of continuing professional development


Desirable

  • Teaching / training qualification
  • Psychological therapy qualification


Knowledge

Essential

  • Knowledge of principles of assessment of people with mental health problems
  • Knowledge of principles of risk assessment and management
  • Knowledge of Mental Health Act 1983 and Mental Capacity Act 2006 and other relevant legislation
