Job summary

Due to promotion, an exciting opportunity has arisen for a Nurse to join the Shrewsbury & Atcham multidisciplinary teams in the provision of Urgent and Planned Care to patients living in the community. We are a proactive & forward-thinking team, who constantly strive for the delivery of excellent patient care and modern ways of working. The multidisciplinary teams provide nursing and therapy services offering an alternative to hospital admission, supporting timely discharge. We enable our patients to realise their potential independence in their most appropriate place of care.

Main duties of the job

You will need to be qualified to RN level, be registered with the NMC and be able to demonstrate current clinical experience in a variety of NHS settings. We are committed to clinical support, CPD & training.

There is a need for excellent communication skills and a dedication to multi agency working; you will need to work flexibly within a busy, demanding environment with the ability to develop new skills and working patterns.

Job description

Job responsibilities

Able to assess, plan, implement and evaluate interventions, liaising with colleagues and partners to ensure an integrated approach to the needs of the service user.

To organise and deliver skilled clinical support work without direct supervision. This will include individual in a variety of settings

Gain valid informed consent and work within legal frameworks with patients.

To liaise with senior clinicians with regard to patient care, highlighting any variations to expected outcomes from agreed treatment programmes. To provide basic verbal and written clinical reports and document patient interventions/findings in case notes in line with professional body and Trust standards

To instruct and educate patients, carers and families on a range of therapeutic activities.

Person Specification

Experience

Essential

  • Previous community experience
  • Post registration general experience
  • Previous NHS experience
  • Experience of working in a multi disciplinary / multi agency health/social care environment.
  • Experience and knowledge of basic administration systems including Microsoft office
  • Experience of successful partnership working arrangements with other services

  • I T skills or willingness to undertake Lorenzo Training
  • Up to date with Mandatory Training
  • Uses evidence based practice
  • Works within own defined competencies
  • Awareness of changing healthcare factors influencing change
  • Demonstrates tact and diplomacy
