Job summary

The post holder must ensure the delivery of education and training to support evidenced based care across and within the clinical teams. S/he will contribute to the improvement of standards to care through the development of all staff groups, participating in audit and research, teaching and supporting colleagues and the provision of effective leadership

The post will involve S/he working 2 days across a 5 day week clinically supporting patients with palliative and EOLC needs including triaging, written and telephone referrals of patients and prioritising. S/he will support the development of generic and specialist staff in generalist palliative care, EOLC

Main duties of the job

Role model standards to care and behaviour through clinical practice.Ensure visibility within clinical areas.Contribute, with the EDP, to formulating palliative and EOLC trust strategy andpolicy. Support the developing role of the practitioner by contributing to the change processes through, leadership, research, education, training and clinical practice. Make clinical decisions working within the 'Scope of Professional Practice' and support the CNS team in identifying those areas of practice most appropriate to be undertaken by the nurse/support staff or others. Bring relevant national information, policies, guidelines and procedures that may impact on service to the attention of the Lead Nurse, Matron, CNS team and MDT. In conjunction with the Lead Nurse, Matron, CNS team and MDT set and monitor standards, promoting best practice in the specialist area.
