The lead specialist dietician will work across 7 days on a rotational basis, across the county as required, as a fully integrated member of the multi-disciplinary integrated community stroke service team(community).
To work as a specialist autonomous practitioner providing advanced clinical expert dietetic support and advice to patients and Health Professionals in the specialist area of stroke care.
To support the band 6 dietician and more junior members of the ICSS team and as this is a new service, to provide leadership in the development of dietetic care as part of the multidisciplinary team.
To lead and contribute to policy development on matters relating to service development for the ICSS Team.
To develop appropriate resources within this specialist area of dietetic practice.
To support education and training across the stroke pathway and to act as a clinical resource for colleagues.
To help to establish Research and Development and a robust Audit review of service within this specialist area of dietetics.

Main duties of the job
- Be a senior member of the ICSS multi-disciplinary team and represent the team at regional/national meetings as appropriate.
- Lead in the preparation and planning of evidence-based diet sheets, teaching material, dietetic literature and specialist services literature.
- Work collaboratively with all members of the team and share information with team members formally and informally e.g. clinical reviews, discharge summaries and provide professional options when necessary.
- Work autonomously as a specialist dietitian and manage own varied and potentially complex caseload in collaboration with members of the ICSS team and other teams across the stroke pathway.
- Complete full assessments of individual's nutritional status, using appropriate anthropometric, biochemical and medical records and identify their nutritional requirements.
- Devise core and specialist nutritional treatment plans e.g. re-feeding programmes, enteral feeding regimens, diabetes, weight reduction, nutritional support, food intolerances, secondary prevention risk factor management.
- Implement specialist group and individual treatments to achieve individual treatment goals
- Identify and manage the risk of re-feeding syndrome in nutritionally depleted individuals.
