Job summary
It is an exciting time for those supporting patients to leave acute hospital care with our discharge to assess initiatives in Worcestershire.
We are looking for a highly motivated and pro-active Nurse to join our Onward Care Team at based at the Alexandra Hospital in Redditch with occasional cross site to Worcester Royal Hospital. We ensure patients get the care that they need, when they are ready to leave hospital, ideally at home first.
This post is for 37.5 hours over a 7 day week including bank holidays. It is for maternity cover for 12 months.
The role will involve working closely with patients, families and the acute hospitals nursing, therapy and medical teams and social care colleagues in a multi-disciplinary, inter-disciplinary way to ensure a safe and timely transfer with our community discharge to assess providers.

Main duties of the job

The role will involve working closely with patients, families, the acute hospitals nursing, therapy and medical teams along with social care colleagues in a multi-disciplinary, inter-disciplinary way. Ensuring a safe and timely transfer to a community hospital or with our community discharge to assess providers, both in and out of county.
The role will involve working with the Continuing Health Care team to facilitate a patients discharge under the fast track process
At times this can be challenging You will be expected to be organized, with excellent communication and IT skills. You will need to be able to work under pressure, prioritizing your workload against constantly changing demands. The service is key to delivering the reduction in delayed transfers of care from acute hospital settings, reducing long length of stay and supporting patient flow through the system
