We are very excited to be advertising this Band 5 (RMN or RGN) post working with people with dementia on either Sage (male) or Rosemary (female) ward Worthing. This is a particularly exciting time to be joining our team, as we have recently reorganised our West Sussex in-patient dementia services which are now all housed within our specialist in-patient unit.

The post-holder will work as part of a dedicated MDT assessing and treating men/ladies with dementia in our 10 bedded dementia ward. The successful candidate will join a friendly, highly-skilled, enthusiastic and supportive MDT. We will be taking a keen interest in helping you to develop the wide range of skills important to this role the post-holder will be encouraged to develop special interests and expertise in line with service and team objectives. You will receive regular managerial/ clinical supervision, as well as a commitment to your own continuing professional development.

Main duties of the job

You will work as part of the multi-disciplinary team undertaking assessments of patients with dementia to develop care and recovery plans in collaboration with the patients and their families/carers.

In conjunction with the team, you will assess and manage risks through assessment regularly reviewing and working in conjunction with the multi-disciplinary teams. You will be expected to administer and monitor medication

You will act as a Primary Nurse providing clinical and managerial leadership on the ward, whilst also supervising junior staff and trainees.

Job responsibilities

The post holder will work as part of a multi-disciplinary team acting as a named nurse for service users. They will:
  • Undertake assessments and reviews of individuals with dementia presenting with higher levels of risk.
  • Develop care plans in collaboration with the service user, family/carers and the MDT.
  • Offer specialist nursing assessment and advice where required.
  • Provide clinical and managerial leadership on the ward.
  • Provide supervision to junior staff and trainees where appropriate.
  • Regularly take charge of the clinical environment over a span of duty.


Person Specification

Knowledge/Experience

Essential

  • Knowledge of MHA (1983/2007).
  • Knowledge of Mental Capacity Act (2005), to include practical knowledge of FACE documentation, DOLS and Best Interest decisions.
  • Child and Adult safe guarding procedures.
  • Demonstrates ability to assess and manage clinical risk
