We are looking for a friendly, approachable, hardworking, caring and enthusiastic Nurse to run our Discharge Lounge, at Eastbourne District General Hospital. You will lead the teams to support and instigate discharges & patient flow, which in return improves timely bed availability at East Sussex Healthcare NHS Trust.
The successful candidate must be able to work all shift patterns that are required for this role, currently 07.00-19.30hrs Monday to Saturday.
Main duties of the job
Responsible for the safe discharge of patients from ESHT
You will have in depth knowledge of complex discharge processes and demonstrable managerial experience.
You will have good communication and interpersonal skills and the ability to work in a calm and methodical manner.
You will lead by example, demonstrating effective role modelling to all members of the MDT
This job role includes daily liaison with the site mangers and all clinical areas, including Emergency Department, AAU and Outpatients Department to support the timely identification of patients for planned discharge and assisting in facilitating their transfer to the lounge whilst waiting for their medications, hospital transport or to be picked up by their family.
Other duties include helping clinical areas to book hospital transport if required.
You will act as a deputy to the Matron, in their absence.
As a registered nurse you will have significant experience in an acute hospital care setting, with extensive knowledge of the discharge process through relevant post-qualification training and experience.
Excellent communication skills are essential for this post as much of the nursing care we provide relates to health promotion. You will need to be able to work effectively and collaboratively with all Trust colleagues.
This is a high-profile role within the organisation and essential to support the Trust in meeting their performance targets.
