The Paediatric Liason Nurse role has recently moved under Surrey and Borders Partnership leadership, offering holistic assessment of young people presenting to acute hospitals, across Surrey, who are experiencing emotional and mental health crisis. We are excited to be able to offer these Band 5-6 development posts within our acute hospitals, with wider support from our Crisis Intervention Services working closely alongside the PLN role.

Following assessment, to get a greater understanding of the young person's need and risk (including safeguarding), you will be able to plan with the young person the support they need currently. This may involve others within Mindworks and the alliance partners, other professionals such as social care, school etc. that the young person and family/carers identify. You will ensure that acute hospital colleagues are updated regarding the young persons care throughout.

You will complete a My Safety Plan, risk assessments and care planning, alongside the young person and with appropriate support, safely facilitate discharge. If a young person requires more intensive support, you will liaise with our Consultants and Hope Service to meet the young persons identified needs.

You will be working closely alongside the CYPS Crisis Intervention Service, an established and supportive team. Within our CYPS Crisis Pathways, we are keen to support further career development and training while providing regular supervision.

Main duties of the job

Provide timely consultations to the ED Team when CYP present in crisis

Working individually and jointly within the Mindworks alliance to provide assessment and treatment to CYP and their families

Undertake assessments including risk of/to the YP/family either individually or, in identified complex cases, in collaboration with the ED Paed Team or CYPS CIS.

Liaise with Social Care, Education, and other appropriate agencies to ensure the best outcome for CYP

Maintain accurate and timely records of all work undertaken

Promote effective liaison with all statutory and voluntary agencies for CYP and MH

Identify own training needs and undertake CPD

Ensure high standards of care are provided including dignity, individuality, self-esteem and wellbeing of individuals and their families are given.

Band 6 (in addition to Band 5)

The post holder will lead clinical assessment of CYP who may have a complex MH needs and present in crisis, working alongside MDT colleagues to ensure that the most effective care and treatment is provided resulting in a safe discharge

Co-ordinate and liaise with Paed ward should admission be required

Work closely with ED, the Adult Psych Liaison Service, MH Crisis Teams, On Call Duty Psychiatrist, On Call Managers, other Paed Liaison Workers, ensuring CYP are seen in a timely manner, risk assessed and a management plan established

Represent the Service at professional/network meetings

Job description

Job responsibilities

My job makes better lives by providing timely assessments and interventions that contribute to the care and recovery of CYP presenting with mental health, emotional and social care needs in the Emergency Department of the District General Hospital.

Job Overview: To be a core member of the multi-disciplinary paediatric service of the Emergency Department and consulting, participating, or leading in the provision of crisis and risk assessments, planning care and bringing together those teams to support CYP who could be discharged home with help and support from the community services or working with the Paediatric Ward to facilitate a short-term admission should this be indicated as a need for the C/YP.

Qualifications required

Degree level qualification in Nursing RMN, LD or RG Children accepted

Experience required

Band 5 minimum of 1 years experience working with people with mental health problems.

Band 6 2 years post qualification experience of working in the community with young people with mental health difficulties and their families. Ability to undertake: Mental Health Assessments, Risk Assessment and implement Risk Management Plans and outcome measures.

Suitable for someone who is passionate about providing high quality, evidence-based Crisis and Liaison Interventions for CYP and who enjoy working within a busy environment with multi-agency professionals.

Job Purpose

Key Responsibilities

To provide timely consultations to the ED Team when CYP present in crisis.

To provide assessment of CYP who present at ED, including offering discovery conversations which may include deliberate self- harm, safeguarding needs etc. and make a safety plan to ensure the right follow up is offered in the community or signpost appropriately.

To provide support, advice and education to the Emergency Departments and Paediatric Wards to build knowledge and skills of staff within these settings in the care and treatment of CYP with mental health and emotional needs.

To case manage children with mental health problems, coordinating between the medical, nursing, CYPS community teams

To work individually and jointly within the Mindworks alliance to provide a specialist, assessment and treatment service to CYP and their families. Home :: Mindworks Surrey (mindworks-surrey.org)

Band 5

Undertake holistic assessments including risk of/to the C/YP/family, either individually or in identified complex cases jointly with another member of the ED Paediatric Team or CYPS Community Team.

To liaise with Social Care, Education, and other appropriate agencies/professionals/ carers to ensure the best outcome for C/YP.

Develop the My Safety Plan before discharging from ED.

Offer support for Students on placement.

Co-work with other members of the MDT and Surrey wide PLN Service.

Take responsibility for maintaining accurate and timely records of all work undertaken in accordance with Trust policies and legal requirements.

Provide statistical information on all activity undertaken, in line with Trust procedures. Have basic IT skills and data entry skills to enter clinical information on the databases.

Contribute to the teams overall standard of services provided.

Promote effective liaison with all statutory and voluntary agencies concerned with CYP and MH.

Through the appraisal and supervision systems identify own training needs and undertake continuous professional development.

Ensure high standards of care are provided, and that regard to dignity, individuality, self-esteem and wellbeing of individuals and their families are given. Monitored and evaluated on a continuous basis.

Band 6 (in addition to Band 5)

The post holder will lead clinical assessment of CYP who may have a complex co-morbid mental health needs and present in crisis, working alongside medical, nursing and other professional colleagues to ensure that the most effective care and treatment is provided resulting in safe, timely discharge

To co-ordinate and liaise with Paediatric ward should admission to Paediatric Ward be required.

Work closely with the Emergency Departments, the Adult Psychiatric Liaison Service, Mental Health Crisis Teams, On Call Duty Psychiatrist, On Call Managers, other Paediatric Liaison Workers, ensuring CYP are seen in a timely manner and risk is assessed and a management plan is developed.

To work within a highly experienced multi-disciplinary team involved in the assessment and treatment of CYP with serious and complex mental health difficulties in crisis.

To manage own workload from referral to discharge/transfer, holding medium to high risk cases as well as ensuring time to attend supervision. Learning sets and other forums are maintained.

Ensure that outcome measures for this service are implemented and audited. Contribute to relevant research and ensure that interventions on the pathway are evidence based.

Be responsible for the initial assessment and subsequent facilitation of referral to Hope and/or discharge with a signed care plan regarding safety and recorded on the EPR.

Report to and advise referrer on assessment and treatment outcomes and respond to requests for and provide formal reports for other agencies.

Represent the Service at professional/network meetings to offer advice/consultation on complex cases.

Develop and implement systems to monitor and evaluate the service

To undertake relevant CPD to improve assessment and treatment skills regarding crisis care.

Be competent in conducting crisis assessments.

Other duties:

To actively formulate a robust My Safety Plans with CYP and their families/carers inclusive of risk assessment and management and positive risk taking.

Contribute to the development and implementation of clinical guidelines, protocols relevant to sphere of responsibility.
