An exciting opportunity has arisen in South East Essex for an experienced band 6 Occupational Therapist to join our vibrant multi-professional community Urgent Community Response Team (UCRT) Admission Avoidance Team. Every member of our team is focused and passionate about ensuring that all our patients receive the highest standards of care, which is delivered with dignity, respect and compassion.

Main duties of the job

Undertake comprehensive, holistic assessments of individuals, including those with a complex presentation, using investigative, analytical & clinical reasoning skills, and formulate a treatment plan.

Undertake functional assessments for patients, by using investigative and analytical skills for those with acute/complex/rehabilitation or palliative care needs.

Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector, to ensure identified needs are met and care co-ordinated appropriately.

Support collaboration of UCRT services across Mid and South Essex, which may include working across boundaries within the three localities.

Communicate effectively with individuals and their carers to maximise rehabilitation and treatment potential, and to aid motivation and understanding throughout the treatment process.

Empower/enable patients and carers to maximise potential for self-care.

Be responsible for ensuring workload for self and others is planned and prioritised according to service targets and needs.

Ensure patient held records are completed, and SystmOne updated on a daily basis.

Carry out safe moving and handling practice for patients with varying needs within a community setting, in accordance with Trust policy

Teaching, advising and providing support to patients and/or carers throughout treatment, illness and rehabilitation to achieve optimum health/independence

Job responsibilities

The post holder will work within the Urgent Community Response Team (UCR) as part of the wider Integrated Care Team to assist in the prevention of inappropriate hospital admissions in line with the overall objectives of the service.

Responsibilities will include, carrying out initial assessments and planning care for patients on the Urgent Community Response Service who are in crisis, in need of rehabilitation and reablement or to prevent hospital admission.

Using evidence-based/patient centred principles to assess, plan, implement and evaluate interventions, as well as responsibility for managing and transferring an individual patient's ongoing care to other professions and agencies as appropriate.

The post holder will work in partnership with patients, carers and other health and social care agencies to promote independence and develop individual plans of care relevant to patient need.

You will be expected to work multi professionally, support other team members roles, contribute to the future development of the service providing professional advice and appropriate training to others in the multi-disciplinary service.

The post holder will work collaboratively with all partners within the Primary Care Network and as part of the PCN Aligned Core Team to meet the needs of the local population using public health information to support service development and a more seamless connection between all Community Partners.

The post holder will be expected to work within a flexible working pattern over a 7 day week.

Person Specification

Application

Essential

  • BSc OT/ Dip OT
  • HCPC Registration
  • Car driver with current UK driving licence with access to a car for work purposes


Desirable

  • Post registration training
  • To have knowledge of a range of physical and medical conditions and their prognosis


Experience

Essential

  • Experience working in a multidisciplinary setting post registration


Desirable

  • Experience of working in a community rehabilitation setting post registration


Skills

Essential

  • Well established knowledge of a range of standardised and functional assessment tools relevant to the client group
  • Excellent time-management skills and ability to support others in this
  • Ability to work independently and to take initiative


Desirable

  • Enhanced knowledge and understanding of the National Guidelines for falls prevention, National Service Frameworks for Older People and Long Term Conditions and relevant NICE guidelines
