We are a high performing organisation with a track record of success. The Community CHD & HF service provide care to cardiac patients across the county. This substantive Band 7 post offers an exciting opportunity for a suitably qualified and experienced Nurse to work within the Specialist Community Cardiology team.

The post covers core hours of 09:00-17:00h The service runs Monday to Friday although there may be provision to moving to 7 day working in the future.

Main duties of the job

The successful candidate will provide a consistent high quality specialist nursing care for patients and families. With significant post registration experience including recent experience in cardiac care. You will need to demonstrate excellent communication and leadership skills and will need to be committed to developing practice. Working collaboratively as part of a team and building effective relationships with colleagues, you must also be able to work on your own initiative.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

The role of the Advanced Nurse Specialist Cardiology requires a suitably qualified and experienced nurse to be able to utilise and expand their knowledge of cardiac nursing care and be able to offer high quality patient centred care, whilst suporting their peers and colleagues. Be able to have a positive impact on care and confidently deal with difficult and complex situations with sensitivity and compassion. Be confident and influence change.

Person Specification



  • Significant post registration experience some of which must be in cardiology


  • Experience of cardiac rehabilitation
  • Experience of health improvement/promotion
  • Audit experience
