Then this could be the role for you - we are looking for enthusiastic Band 6 Nurse to join our well established and successful team to engage with care home managers to develop and sustain competence of care home staff to facilitate person centred care in order to improve quality and, where required, to utilise clinical skills to educate and support staff in care homes.

The Care Home Clinical Practitioners work within the Integrated Care Division, WVT and in close partnership working with Herefordshire Council Quality Assurance Team to ensure the successful delivery of clinical support and advice to Residential and Nursing Homes across Herefordshire and to provide direct care delivery training and support to care homes.
Main duties of the job
The Care Home Clinical Practitioners duties include providing specialist clinical advice and direct clinical care where required to support care home teams and individual care home staff on a range of quality improvements and ensuring that any interventions implemented are in line with best practice.
The role will link with WVT Tissue Viability, Herefordshire Council, PCNs and care homes to provide the gold standard education and support in wound care

- Live healthcare professional registration (NMC, HPCP)
- Educated to degree level or equivalent level of experience of working at a similar level in specialist area
- Post graduate teaching qualification or experience of working at a similar level

- Able to demonstrate recent clinical practice and knowledge in a healthcare setting
