Dorset Healthcare is working in partnership with Poole Central Primary Care Network (PCN). The PCN is looking for a Dietitian with a broad range of skills and clinical experience with a particular interest in obesity and gastrointestinal conditions, such as IBS.

This is a chance to develop leadership and service development skills to assist the PCN to develop effective and successful models of care across the system. The role is full time but part time/job share will also be considered.

As an experienced and highly skilled Dietitian you will need to demonstrate leadership skills and the ability to be flexible and adaptable to a developing agenda in primary care.

You should have an understanding of Population Health Management to enable the PCN to see how this dietetic role can be used to maximum effect for its patient population. You would work with key members of the Primary care team to establish services that complement mainstream service provision and address the local need.

Working in primary care it is expected that you will build strong partnerships with acute, community and local authority services. Connections with specialist dietetic services within the county, will aid your own CPD but also to ensure that you are linked to established patient pathways and are well placed to signpost to existing services.

Main duties of the job

The post holder will work as a highly autonomous practitioner, often acting as a first contact practitioner within a primary care multi-professional team.

To improve the patient experience of access and waiting times

To use their competencies to manage parts of the primary care workload and save GP time, with a focus on weight management and GI conditions.

Improve the quality of care and implement NHS Long Term plan goals.

To manage change, to monitor and improve standards through supervision, evidence based practice, clinical audit, research and education. To promote and demonstrate best practice by integrating evidence into practice

Provide training to the wider multidisciplinary primary care team to raise nutritional awareness and promote the benefits of dietetic services with respect to a wide range of diet-related conditions within the PCN.

The post holder will be willing to work flexibly across seven days if required including weekends and bank holidays.

Person Specification

Knowledge, Skills and Training

Essential

  • Degree or equivalent in Nutrition and Dietetics leading to
  • HCPC Registration as a Dietitian
  • Membership of the British Dietetic Association
  • Knowledge and understanding of Primary Care Strategy relevant to role


Desirable

  • Non-Medical Prescriber (if professionally appropriate)


Job Specific Experience

Essential

  • Experience at Registered Practitioner Band 6 level or above
  • Able to demonstrate specialist expertise and wide ranging knowledge underpinned by theory acquired through CPD in relation to clinical assessment, treatment and evaluation.
  • Evidence of risk management skills and experience.
  • Evidence of involvement in meeting relevant clinical governance objectives.
  • Experience of multi-disciplinary working.


Desirable

  • Able to demonstrate publication, presentations and personal research at local and regional events.
  • Evidence of involvement in research

Additional Requirements

Essential

  • Subject to the provisions of the Equality Act, able to travel using own vehicle on Trust business.
  • Demonstrable skills in written and spoken English to a standard which enables the post holder to carry out the full range of duties and responsibilities of the role effectively.
