We are seeking to appoint a dedicated and motivated Health Visitor to work within the South Derbyshire and Derbyshire Dales locality.
Various hours are available (full time and part-time posts are available)
The South Derbyshire and Derbyshire Dales 0-19 Team are a Gold accredited locality for their work with Families and Communities.

The 0-19 Children's Service was awarded DCHS team of the year in 2019.
Working in a diverse town you will find that there are lots of opportunities for developing public health work with communities and partner agencies, as well as delivering the core contacts within the Healthy Child Programme.
The right candidate will hold the SCPHN qualification in Health Visiting and the V100 and be an innovative and dynamic team player who is looking for a new challenge.

To deliver the public health role as a SCPHN Health Visitor working with communities and partner agencies and delivering the core contacts within the Healthy Child Programme.

During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.

Main duties of the job
To deliver the public health role as a SCPHN Health Visitor working with communities and partner agencies and delivering the core contacts within the Healthy Child Programme.
- To provide health visiting services to families in the community team caseload by delivering the Healthy Child Programme.
- To manage a defined active caseload and take responsibility for Safeguarding Children issues within that caseload.
- To have continuing responsibility for the assessment of individuals needs and the development, implementation and evaluation of programmes of care.
- To prescribe according to assessed individual need from the Nurse Prescribing Formulary.
- To manage a team of health visiting support staff.
