The successful candidate will need a minimum of5 years post registration experience, be able to function as anautonomous practitioner with expert clinical skills and a good knowledge of long term conditions. It is also important that the candidate can demonstrate flexibility, good communication skills, an ability to work as a team member as well line manage the Assistant Practitioners within the team.
Main duties of the job
The post holder will work as a case manager, under the supervision of the Community Matron. You will be involved in delivering integration, working with GPs, Social Services, Care Coordinators and the Community Nursing/Therapy Services. You will be providing case-management, care co-ordination and support, to patients in their own homes. This input will help to avoid unnecessary hospital admission, reduce hospital bed days and maintain health and well-being for the patients. We also work closely with our Acute sector going into to the local Hospital to facilitate early discharge.
This post requires that you work flexibly acrossall Localities in Haveringwithin the Integrated Community Team.
