Job summary
Job title: Senior Clinical Fellow in Anaesthesia
Location: Harefield Hospital
The work of the department comprises anaesthesia for all forms of cardiac and thoracic surgery, the care of patients in the Intensive Therapy Unit, anaesthesia cover in the X-ray/Cardiology Department for diagnostic, electrophysiology and interventional/therapeutic procedures (including TAVI), and \stand-by cover for heart biopsies and angioplasties. There are three cardiac and two thoracic operating theatres. Cardiac and thoracic lists run every day. There is an increasingly active artificial heart and ventricular assist device program. An active cardiac and pulmonary transplant programme impacts on night work and ITU cover. Acute and chronic pain services are provided by the department.
The range of conditions treated is wide providing varied, interesting and valuable experience for anaesthetists.
Main duties of the job
This is a highly specialised post and would be ideally suited to an NTN holder in year 4 or 5 of their training (as out of programme experience) who wishes to pursue a career in cardiothoracic anaesthesia and intensive care. The appointee should have a minimum of four months experience in cardiothoracic anaesthesia, and consideration be given to other suitable candidates including those already in possession of their CCST or appropriately qualified overseas trainees.
This post is for 6, or preferably, 12 months, subject to satisfactory performance and the training needs of the individual Clinical Fellow. In general, the year will be divided into modules covering anaesthesia for adult cardiac surgery, thoracic surgery and cardiothoracic intensive care, with opportunities for research and skills in TOE. There may be opportunities to join the transplant retrieval team.
About us
The Department of Anaesthesia and Critical Care has undergone a restructuring process to separate junior medial posts into those based in anaesthesia and those based in critical care. There are 21 juniors at registrar grade and 9 CT1/2 posts. This allows the provision of three separate on-call rosters - one for anaesthesia and outreach, and two for the intensive care unit. The junior staff comprises of:
7 Specialist Registrars (Deanery rotations in anaesthesia; 5 posts incorporate 3 month blocks in intensive care)
7 Clinical Fellows (intensive care only)
7 Clinical Fellows (based in anaesthesia but incorporating 2-3 month blocks in intensive care)
9 CT1/2 posts (including 1 Deanery CT rotation, 2 cardiology/ICU rotation) - intensive care
Job description
Job responsibilities
The post holder will be assigned to one or more blocks of 2/3 months in adult intensive care. When attached to the adult intensive care units the Clinical Fellows are expected to attend the ward rounds, participate in the daily clinical duties and attend the relevant teaching programmes.
The Clinical fellows participate in the provision of all aspects of anaesthetic and intensive care for adults undergoing cardiothoracic procedures. This includes preoperative assessment, intraoperative care and postoperative management. This may be carried out under the immediate or more distant supervision of the consultant staff as appropriate.
