Community services are expanding in line with the NHS 10 year plan to provide more care in the community and so this is an exciting place to be at this time. Community nurses provide an essential role to deliver care to patients in their own homes, avoiding hospital admission and allowing people to stay in their homes longer.

Main duties of the job

We are seeking motivated, dynamic, and passionate Band 5 Community Nurses to work within our new Rapid Assessment Team. You will be working across the Bedfordshire, with your base locality to be discussed at interview. This is an excellent opportunity to develop your knowledge and skills within the community nursing setting. You will have the opportunity to be involved in facilitating self-care for patients with long term conditions, health promotion, assessment and delivering an urgent response to those in need. You will also be involved in innovative activities and projects that enhance patient service delivery and staff wellbeing.

Experience in the field of adult community services is desirable but a willingness to learn and contribute is more important. Consideration will also be given to staff who are newly qualified and are passionate about a career in community nursing.

Job responsibilities

You must have a driving license and have access to a vehicle in order to carry out your duties. You will be required to work a shift pattern covering 7 days a week from 08:00-20:00.

Person Specification

Education/ Qualification/ Training

Essential

  • Registered General Nurse
  • Demonstrate evidence-based practice and up-todate clinical skills.


Desirable

  • Evidence of post-basic professional development
  • Relevant teaching/mentorship qualification or commitment to undertake
  • Post-registration nursing experience


Experience

Essential

  • Evidence of and commitment to multiprofessional working.
  • Understanding of the interface between primary and secondary care.
  • Comprehensive understanding of the roles of others in health and social care


Knowledge and Skills

Essential

  • Literate in IT/Computer Skills
  • Evidence of knowledge of clinical governance
  • Knowledge of the principles of Safeguarding of Vulnerable Adults and Children.
  • Knowledge of the principles of health promotion and prevention of illness.
  • Good oral communication skills based on fluency in English language


Desirable

  • Demonstrate an understanding of national government initiatives in health care.
