An exciting opportunity has arisen for a Band 7 post as a Diabetes Specialist Nurse Team Leader to join our dynamic Diabetes Inpatient Care and Education team (DICE) at GUH.

Main duties of the job

  • Act as advocate for patients and healthcare professionals.
  • To provide professional, clinical, and operational leadership and support to healthcare professionals.
  • To formulate and deliver training and education for healthcare professionals.
  • To deputise for the Diabetes Senior Nurse when required.
  • Responsibility for the formulation and management of clinical action plans.
  • Responsibility for clinical audit.
  • To provide leadership and support within primary/secondary care with the delivery of diabetes care.
  • To maintain and promote accurate record keeping and documentation following national and professional standards and local protocols and policies.
  • To collect and input clinical data to clinically based computer systems: to access and utilise all available clinical information to support care planning and patient management.
  • To ensure effective and safe medicines management processes are adhered to.
  • To plan and provide education for patients with diabetes, to enable them to self-manage their condition.
  • Ensure awareness of sources of support and guidance, and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate.
  • To take responsibility for the day-to-day operational management and clinical practice.

Job description

Job responsibilities

The post holder will be part of a dynamic inpatient diabetes team, working alongside the Diabetes Nursing Team, Endocrine Consultants and wider multi-disciplinary teams to provide leadership for the DICE team, participating in daily ward rounds, formulating treatment management plan , providing education to patients in preparation for discharge, providing foot assessments and referring to other multidisciplinary team.

The post holder will be providing education and training to patients, their family and all health care professionals.

They will be operationally responsible for their practice within secondary care settings.

Person Specification

Qualifications

Essential

  • NMC Registered nurse with minimum of 18 months post registration
  • Non medical prescriber or working towards


Desirable

  • Teaching qualification
  • management training


Experience

Essential

  • Experienced Diabetes Specialist Nurse
  • Staff management experience


Desirable

  • Experience working between primary/secondary care
  • experienced with audit and research process
