Job summary

- Supporting and progressing the development of pharmaceutical services and the integrated medicine management agenda in the UHB by ensuring safe, evidence based and cost-effective primary care prescribing within a locality or cluster group of GP practices.- To ensure safe, evidence based and cost-effective prescribing within a locality/cluster group of GP practices.- To support the GP/cluster and UHB primary care plans.- To contribute to the UHB's prescribing systems and processes as part of the Prescribing Team.- To act as an Independent Prescriber or have the ability to study and qualify as an independentprescriber.- To contribute to the training of pharmacy trainees within GP practices

Main duties of the job

An exciting opportunity has arisen for an enthusiastic and self-motivated pharmacist to work in a developing cluster pharmacist role within the Primary Care team in Cwm Taf Morgannwg University Health Board. The successful applicant will be part of a team of advanced pharmacists, technicians and other staff, working in GP practices in the Bridgend East locality of the Health Board. The Medicines Management team will provide you with support to integrate into the practices, where you will provide medicines optimisation support, advice and facilitate the training of new pharmacy staff.

You should have excellent communication skills and the ability to work effectively with other Health Care Professionals and Primary Care staff.

You will have opportunities to undertake the Pharmacist Independent Prescribing course and will be supported to develop both clinically and strategically.

This post is fixed term for 18 months due to the needs of the service
