We are seeking an experienced Occupational Therapist to join us. The Basingstoke Community Therapy Team is a friendly and supportive team of Community Therapists and Therapy assistants providing a domiciliary rehabilitation service to patients in North Hampshire. You will develop your expertise treating a range of problems related to elderly care, falls, palliative care, orthopaedic, neurological and respiratory conditions. You will also have opportunity to develop your leadership and management skills within the team.

This post would suit an established B6 therapist or an experienced B5 with an interest in community working. Successful applicants will need to be an efficient worker, a good communicator and be confident working as an autonomous practitioner. Community experience is desirable. You will be responsible for managing your own caseload whilst working effectively within the multidisciplinary team to ensure an integrated approach to patient care - preventing unnecessary admission to hospital or long-term care, promoting functional independence and facilitating timely discharge from hospital.

Southern Health NHS Foundation Trust is committed to improving patient experience and clinical outcomes by looking at new ways of working. There are great development opportunities through our trust with CPD and the option to undertake MSc modules. You will of course receive good support and professional development from your colleagues too.

Main duties of the job

To provide advanced clinical and professional Occupational Therapy to the Community Therapy Team, and to provide support to the Integrated Community Care team leaders in ensuring that the team contributes to the implementation of Southern Health Clinical Governance Strategies.

Undertake Occupational Therapy assessments of patients with diverse and complex conditions, providing a clinical diagnosis (where appropriate), development, and delivery of individualised treatment programmes.

To support the case management process by providing expert clinical advice and therapy to patients with a variety of complex conditions living in the community who are at risk of deteriorating health that may result in declining quality of life or avoidable hospital admission to secondary care

As a supervisor, delegate tasks to assistant practitioners and junior therapists relevant to their skill level and competence.
