Job summary
The Surrey Integrated Service for Young People [HOPE Service] operates from two bases, East Surrey [Epsom] and West Surrey [Guildford] and provides a Service for young people aged 11 to 18 years old presenting with severe mental health, behavioral, emotional and relationship problems. The Service is the link between CYP (children and young people) Services (as part of the Mindworks Alliance Group, Surrey Families Directorate [Social Care and Education] and the Specialist inpatient services. It offers support to CYPS Services such as CYPS Community Teams, Crisis Pathway Services, and Surrey Families Directorate, through structured Day Programme and Outreach Teams working in community settings. The Service also provides transitional support for those young people in hospital in the process of returning to their local community. The Service considers referrals for admissions to adolescent inpatient facilities and from the CYPS Community Teams as well as Childrens Services Teams and advises of the best appropriate care; this ensures that young people get access to the right level of treatment.
In this role you can be based either in Epsom or Guildford to cover the East or West Surrey catchment area and this can be discussed at interview.
Main duties of the job
Suitable for someone who has effective active listening skills (questioning and summarising), who can use good use of empathy and diplomacy, build good rapport, knows their own limitations and has good time management.
As the Band 7 Senior Nurse you would have a small defined caseload as well as the opportunity to lead new assessments. You would have the opportunity to supervise and develop junior staff and undertake the preceptorship for newly qualified nurses which has been tailored particularly for nurses wishing to work in children and young peoples services. You would be part of the appraisal process for anyone you are supervising.
This is a great opportunity to not only work with young people during their most vulnerable time, but to be part of the multi-disciplinary and multi-agency approach to meeting their needs. You would be working alongside health staff such as Psychiatrists, Drama and Art Therapists, Psychologists and other nurses as well as Social care staff and Teachers. The Hope service is unique in its offer for young people in Surrey and uses a DBT model of care to faciliate their journey of recovery.
We offer training opportunities that are tailored to your pathway and career development including modules concerning assessment and risk assessment of adolescents, DBT-A, DBT skills as a basic introduction. We are keen to explore other developments with you whether it be NMP, ACP or other interests and will support you on this journey.

Experience required/ Key Responsibilities
To manage a caseload of young people who are vulnerable and can be deemed as high risk with complex mental health and social care needs.
To provide assessments and reviews within community, acute hospital or Health Based Place of Safety (HBPoS) to support decision making with regards to hospital or respite and options regarding diversion from tier 4 provisions.
To provide comprehensive mental health assessments focusing on risk management of young people who are at risk of a hospital admission and that may include family/placement breakdown.
To recognise potential crisis situations and act responsively and responsibly to resolve the situation, seeking assistance if required.
To provide assertive outreach to young people and their families/carers to monitor mental state and risk and help re-integrate into an educational setting.
To promote shared decision making with Young People/families/carers and be involved with all aspects of care.
To monitor effects and side effects of medication and report appropriately to the prescribing Consultant, and Clinical Nurse Lead.
To ensure documentation is up to date and reviewed in a timely manner according to the Quality Standards for CYPS e.g care plans, risk assessments, organising of care plan meetings and in accordance with NMC guidance regarding good record keeping.
To develop and maintain effective relationships with Mindworks Alliance, SCC and other partnership agencies.
Develop creative packages of care for young people and their families/carers to try to prevent hospital admission or shorten length of stay when admission is necessary.
To be a resource for specialist advice and information to other colleagues, agencies and organisations of adolescents with complex mental health issues.
To provide evidence based interventions to young people and their families/carers in accordance with NICE guidelines.
To uphold the principles of the NMC code of professional conduct at all times.
General
