In order to successfully complete development into this role it will be necessary to fulfil a detailed development plan which will be designed on an individual bases, following recruitment.

A candidate successful for development will be paid a band 7. Only on completion of the development plan will the successful applicant be confirmed into the position outlined in the advertised Job Description.

If the successful candidate already meets the essential criteria of the higher banded role, they can commence as a band 8A.

Jean Bishop Frailty Community Services (JB-CFS) have been operational since May 2018, and in that time provided innovative, anticipatory patient tailored care to frail residents of Hull and the surrounding areas, including those residing in care homes. The Centre has already been recognized for national awards and continues to receive outstanding patient feedback. The JB-CFS also featured on NHS England's website dedicated to the NHS Long Term Plan as the leading case study in the Ageing Well section. More recently, the team featured on a Panorama programme which assessed the innovative and new ways of working delivered by our team, evidence to support not only our frail population but also the demands placed on the NHS.

Main duties of the job

The team working at the JB-CFS feel privilege to be a part of delivering transformed care for older people in Hull.Our workstreams include:-o Provision of comprehensive Geriatric Assessments from both within the Integrated Care Centre and Residential Care Homes located across Hull and East Ridingo Medical support to several community bedded units, used as a step up/down facility to support hospital admission prevents and the discharge to assess model (D2A)o Advice and Guidance line accessible to Healthcare Professionals involved in the care of a frail patient including Care home staffo Urgent Community 2 hour Response

This is an amazing varied opportunity for an independent prescriber with advanced practice qualifications who is passionate about caring for older and frail patients, who also has the drive to help further develop a high profile service care within the community. In this role you will work as an expert autonomous practitioner across all work-streams supported by the JB-CFS assessing and pro-actively planning the care and treatment of older people and those with frailty, working closely with the multi-disciplinary team and community services to ensure patients receive holistic and comprehensive care and support. You will need to possess the necessary skills to deliver services to patients with a wide range of complex clinical needs, including acute presentations.

The abilities to communicate professionally and effectively whilst in a caring manner with patients and their carers together with experience of working within a multi-disciplinary team are also essential, as is working experience/knowledge of local pathways and referral procedures.

We can offer flexibility to the successful candidate in terms of either full or part-time working. The vacancy is permanent due if preference for a secondment opportunity this too will be considered.

The service is open 365days per week with expectation for successful candidates to be included on weekend and bank holiday rotas

Person Specification

Qualifications

Essential

  • Autonomous Practice
  • Registered with either the NMC or HCPC
  • Relevant nursing/health degree
  • Mentor/teaching qualification


Knowledge

Essential

  • Evidence of extensive expert knowledge in older people
  • Knowledge of strategic issues which influence area of practice at a local and national level
  • Clinical governance issues in community/primary care


Experience

Essential

  • Significant post registration experience
  • Experience of student/staff development/coaching
  • Experience of working within a multidisciplinary team environment
  • Management of frailty and long term conditions
  • Compiling protocols and clinical guidelines
  • Clinical Leadership in support of development of team members
