Job summary
Joined Up Care Derbyshire (JUCD) has been awarded funding for an exciting opportunity to assist in the transformation of cardiac rehabilitation services in Derbyshire to enable an increase in capacity for heart failure patients to access cardiac rehabilitation services. The cardiac rehabilitation services in Derbyshire meet regularly and are working collaboratively to provide equitable services across the system for our local population.
We are looking for a nurse and physiotherapist/exercise instructor with understanding and experience in the delivery of cardiac rehabilitation to use their clinical knowledge, investigative, problem solving, and leadership skills to help transform processes to incorporate more heart failure patients into the cardiac rehabilitation pathways across Derbyshire. This will necessitate participating in the delivery of cardiac rehabilitation services across the Derbyshire sites as well as spending time exploring potential opportunities and new avenues for the delivery of cardiac rehabilitation so increasing capacity within the system.
Staff are required to work across Derbyshire although they will be hosted by Derbyshire Community Health Services NHS Foundation Trust.
Main duties of the job
Using their experience and knowledge, the post holder will assist in the review of present service delivery and explore and investigate opportunities within health and social care settings to deliver effective cardiac rehabilitation in differing ways that would enable increased capacity for heart failure patients within existing service structures. This will require regular rolling meetings to feedback and action plan next steps with service leads across the Derbyshire cardiac rehabilitation services.
The post holder will also assist in providing current programmes of cardiac rehabilitation consisting of individualised exercise prescription to patients with a wide variety of cardiac conditions such as myocardial infarction, cardiac surgery, angina and heart failure offering advice, education and support to patients, carers and other health care professionals in the management of the patient's heart condition.

- Telephone triage patients
- Offer initial support and advice post discharge
- Assess patient's pre and post cardiac rehabilitation in locally held clinics and onward refer to other services as necessary.
- Run the cardiac rehabilitation programme with other team members
- Provide education presentations on a variety of topics aligned to heart health.
Work collaboratively with cardiac rehabilitation services within the system (Joined Up Care Derbyshire) reviewing their services and structures:
- Explore other services locally and nationally and consider different and/or new strategies to effectively deliver services
- Provide Feedback on opportunities and transformations that will enhance the opportunities of cardiac rehabilitation for patients enabling capacity for increased numbers of heart failure patients to access the service
- Meet regularly with team leads to action plan next steps
- Support and deliver new initiatives and assess their impact

Professional Qualifications/training
Essential
- Registered Nurse or physiotherapist or exercise instructor who is either: BASES Certified Exercise Practitioner or BASES Accredited Sport and Exercise Scientist or REPS Level 4 Cardiac Disease (Rehab) qualification in exercise and fitness
- Evidence of recent training relevant to cardiac rehabilitation

- Educated to or working towards degree level training
